Following the events of the prequel, Satya, Abhijith (also known as Shiva), runs an orphanage for the downtrodden while secretly siphoning off black money. When a nine-year-old girl named Jaanu, who suffers from Brugada Syndrome, needs life-saving treatment, Satya arranges her care overseas in Warsaw. There, he crosses paths with Dr. Priya, and with Jaanu’s trust guiding them, they form a fragile bond built on mutual care and shared secrets. The city becomes a backdrop for both healing and danger as Satya grapples with his double life.
In Warsaw, a pharmaceutical mogul named Devendra hatches a dangerous plan: unleash a virus and sell a cure at an exorbitant price to reap enormous profits. Satya consults the doctor and sets a date for Jaanu’s operation, balancing the pressures of saving a life with the weight of his own criminal activities. After a meaningful day, Satya and Jaanu visit the Aviation Museum, a quiet moment that contrasts with the peril lurking in the shadows, and then they return to India.
Back home, a masked figure called Ghost carries out the heist of the Crown Jewel, a priceless piece that Devendra had purchased and expected to receive within a week. A tense chase ensues between the police and Ghost, who manages to slip away once again. The investigation draws in two Interpol officers, Sharath and Kangana, who scrutinize CCTV footage and spot that the masked man is a doppelgänger of Satya. The footage also shows the intruder reciting a Sanskrit mantra during the heists, and the explosions appear to be deliberate diversions to mislead the authorities. Their inquiry leads them to Bangalore, where they learn about ex-ACP Kishore and question him for clues about Satya’s murky past.
Kishore’s revelations set off a cascade of truths: Devendra learns of Satya and assigns his assassin Michael to finish him and recover the Crown Jewel, but Satya kills Michael instead. Sharath and Kangana arrest Satya and press him for answers; in a bid to deflect, Satya feigns guilt and asserts that Kishore was the mastermind behind the plan. Priya, heartbroken by the unraveling deceit, takes Jaanu with her to Poland for treatment. The investigation intensifies as Sharath and his team escort Satya and Kishore to Poland, where they’re briefly detained before being released; a twist of fate sees Ghost aiding Satya and Kishore to slip away and reach the hospital where Jaanu is admitted.
Inside the hospital, Anantha Krishna, a surgeon, attempts to sedate Satya, but the desperate man escapes and convinces Priya and Kishore of his innocence. Satya then exposes Anantha Krishna’s involvement in Ghost’s criminal activities, showing the doctor’s phone with a message from Ghost ordering a meet-up at a pub. They track the clues to a pub, but the Interpol closes in; Ghost distracts them and guides the group to his hidden lair, where they encounter Anantha Krishna and Ghost once more. Anantha Krishna then reveals the startling truth: Ghost is Shiva, Satya’s twin brother.
Past echoes through the present. Durga, an HDO officer and the mother of Satya and Shiva, opposed Devendra’s methods and his virus-and-medicine profit scheme. Devendra kills Durga, and the two brothers are born in the tragedy’s shadow. Satya arrives healthy, but Shiva bears severe nerve damage from the attack in the womb. Their father Prakash returns from abroad to take them away, while Anantha Krishna decides to take Shiva to an ashram for healing and later to Poland for higher studies. Anantha Krishna later shares the incident with Shiva, who vows to avenge their mother’s death and learns of Satya’s ongoing operations, planning to bring Satya to Poland to reveal the truth.
In the present, a fresh crisis hits when Jeet, Devendra’s son, kidnapps Jaanu from the hospital. Satya tracks Jeet with a tracker planted by Shiva, reaches him, and saves Jaanu by destroying the oxygen tank in the ambulance, triggering a deadly explosion. Shiva/Ghost and Kishore are captured by Interpol, only to be kidnapped by Devendra’s henchmen. Shiva/Ghost reveals himself to Devendra, eliminates him and his associates, and frames Kishore for the murders, leading to Kishore’s arrest. Satya is released and returns home with Jaanu, while Shiva uses the wealth from selling the Crown Jewel to fund cures for those harmed by Devendra’s fraudulent medicines.
