On October 22, 1981, Terry, Nancy and Gloria drive to a music festival in rural Washington. They smoke marijuana in the car and hear a radio report about two missing teenage girls. A sudden storm hits, and Nancy crashes, leaving all three unconscious.

Terry wakes to find she and her friends alive and sheltered inside an isolated mansion owned by Marion Penrose. Gloria is injured, and Marion suggests they spend the night until Gloria can recover enough to leave. The house’s eerie quietness sets a claustrophobic mood.

The trio attends dinner with Marion and her elderly mother, while Mrs. Penrose’s mother rails against men. The conversation hints at dark secrets in the house and past grievances. A mysterious man is seen watching the family through the windows, heightening the sense of danger.

Terry finds a human tooth under her bed and later hears heavy breathing upstairs. She searches the attic and spots black-and-white photos of two children and an old tool belt with a handgun and machete. She sees a man staring in through the window and screams in fear.

The next morning, Terry and Nancy shower while someone spies on them through a peephole in the wall. Nancy decides to walk to the local village, venturing into the woods toward the road. The house’s tension continues to mount as the day unfolds.

Nancy travels through the woods toward the village, but a cloaked figure with a long scythe ambushes her on a rural road and murders her. The disappearance escalates the sense that the house holds deadly secrets. Her scream fades as she is attacked.

That same evening, Gloria regains consciousness only to be attacked in her room. A hatchet ends her life as Terry remains unaware of the lurking danger. The survivors’ nerves tighten as the numbers dwindle.

Terry leaves Gloria’s room and is chased by Carl, the Penroses’ disturbed brother. She hides in a shed and discovers the corpses of her friends along with other dismembered bodies. Carl breaches the shed, forcing Terry to flee back toward the house.

Terry escapes into the attic, retrieves a handgun, and shoots Carl in the head, killing him. Marion rushes upstairs in response to the noise, but the house’s secrets are far from resolved. The confrontation marks a pivotal turn in the house’s deadly history.

Marion confronts Terry, chastising her for killing her brother. In a shocking twist, Marion reveals she is actually Mrs. Penrose's second son, Carl’s younger biological brother who dresses as a woman. The family’s true identity unravels in a moment of chilling confession.

Terry attempts to flee but Marion overpowers her, stabbing her to death while raving about gender dysphoria and her obligation to care for her brother and mother. The killer’s fixation exposes the house’s darkest motive and the cost of its secrecy.

As Terry bleeds out, Mrs. Penrose calls for Marion from downstairs. Marion answers in a feminine voice, 'No, mother,' revealing the final deception and sealing the house's legacy of madness.
