A decade ago, Max's older foster brother dies by suicide, an event that threads through his choices and identity. The loss leaves Max with a hardened outlook and a hunger to express himself. It also foreshadows the darkness he will encounter later.

As he begins his duties, Max befriends several residents and becomes connected to the everyday workings of the home. Soon, eerie incidents emerge: residents break into his room at night, some bleed spontaneously, and others show unsettling psychological symptoms. The fourth floor remains off-limits, adding tension to his growing curiosity.

Max learns that the fourth floor is strictly off-limits, a rule that only fuels his desire to understand what lies there. The off-limits area houses residents who are listless, suffer extreme bodily harm, and display psychotic symptoms. The mystery of that floor thickens the atmosphere around the whole home.

Max ventures onto the fourth floor and is attacked by a resident who embodies the floor's danger. A staff member warns him to stay away and not to return. The confrontation proves the floor hides something deadly and powerfully wrong.

One of Max's friendly residents plummets from the home and dies, impaled on the fence in a shocking, brutal image. The event devastates Max and pushes him toward a deeper investigation. The incident confirms that the home conceals lethal secrets.

Driven by guilt and curiosity, Max begins a private investigation of the home and its residents. His pursuit of truth leads him back to his foster parents' house, where he hopes to find answers. The investigation sets the stage for a larger conspiracy to unfold.

Max is placed on the fourth floor so the cult can harvest his youth. His brother drains fluid from the remaining victims and himself, and he feeds Max their fluid, which endows him with renewed stamina. With this new power, Max prepares to strike back at the cult.

Max uses the regained strength from the fluid to kill the entire residential cult. After the storm passes, he and the drained victims, who have reclaimed some power by taking back the fluid from the dead, escape the home. The closing scenes leave Max free but forever marked by the horror he faced.
