Its members—Max’s foster parents, the senior residents and even the doctor—have been using the facility to harvest the “nectar” that supposedly resides in a tiny sac behind a person’s right eye. By draining this essence from a young host they can keep their own bodies alive for more than a century.

The revelation hits Max hard when he sneaks into the hidden worship room in his foster family’s house and finds photo albums documenting a yearly sacrifice that has been going on since 1997. One of the victims listed is his brother Luke, who he had always believed died by suicide. In reality Luke was the first young man the cult turned into a living trophy, and the frail, almost‑silent patient Max later meets on the fourth floor is none other than Luke, aged and crippled by the same procedure.

At the climax Max is drugged and awakens strapped to a chair, dressed in a white suit, as the cult prepares to make him their next offering. He is taken to the forbidden fourth‑floor ward, a dim room where dozens of other victims lie barely conscious, their eyes swollen from the extractions. Just as hope seems lost, Luke summons the last of his remaining strength, pulls a spoke from his wheelchair and uses it to gouge out the eyes of the other prisoners, collecting the faint glimmer of youth that still drips out. He hammers and hacks his way through the staff, killing the cult leader Lou, the mad scientist Dr. Sabian, and Sabian’s wife Gretchen in a brutal, blood‑soaked showdown. His foster parents try to flee, only to be crushed by a sudden tornado that tears the roof off the building and brings them down in a single, ironic strike of nature.

With the perpetrators dead, Max frees the other survivors on the fourth floor. Though they cannot regain their lost youth, the injected nectar lets them stand and walk again.
