From the moment Soo-Ah is taken downtown and coerced into a psychiatric hospital, the facility’s brutal regime is laid bare. The director, a figure of ruthless authority, enforces obedience through fear, withholding answers, and a steady stream of “medication” that doubles as control. As Soo-Ah endures the chilling indifference of the staff, she quietly documents each act of cruelty in a journal, determined not to let the truth disappear into the walls of the institution. The story captures the claustrophobic weight of confinement, the mental toll of mistreatment, and the slow, careful unraveling of a system designed to weaponize vulnerability against those who are supposed to be helped.

A botched escape only deepens the hospital’s darkness, and the consequences become brutally personal. An orderly named Dong-sik, weary of the inhumane treatment, reaches out to Soo-Ah’s boyfriend, Lee Woo-Jin, to mount a rescue. The plan fails in a devastating way: the trio is captured, Woo-Jin is subjected to a horrific disembowelment, and the rescue ends in tragedy. In the chaos, Mi-ro, another patient who is trapped in Hyung-sik’s web of power, unintentionally sets the director’s office on fire, triggering a chain of events that exposes cracks in the hospital’s facade. Mi-ro helps the escapees reach freedom, but Dong-sik remains behind to pull others out when the moment calls for courage.

The escape route is brutal and costly. As Soo-Ah and Woo-Jin break free, Hyung-sik—desperate to recover his illicit earnings—lunges after them, dragging Woo-Jin into a fire that seals their fates. Soo-Ah makes it out of the inferno and returns to her home, only to confront another layer of horror: her abusive step-father is found dead from a gunshot. The once-stable world quickly tilts into suspicion and danger, and Soo-Ah is arrested on charges of murder and arson tied to the hospital blaze.

A year passes, and the focus shifts to Na Nam-Soo, a programming director who had been sidelined for exposing political corruption. He is drawn into the mystery when he is tasked with uncovering what really happened to the burned hospital. He discovers Soo-Ah’s diary, but the trail doesn’t clearly point to the fire, so he pursues Soo-Ah directly. Now incarcerated on murder suspicions and increasingly uncooperative, she becomes the focal point of Nam-Soo’s investigation. As he and his crew return to the ruined hospital, they encounter a severely burned Dong-sik, whose reactions hint at deeper, hidden truths. Through relentless questioning, Nam-Soo uncovers a tangled network: the hospital is a front for an organ trafficking ring and a broader scheme of disappearances, operating by exploiting a legal loophole that allows families to authorize psychiatric detentions without a clear diagnosis.

Nam-Soo’s exposé ignites public outrage and pressure, forcing Soo-Ah to confront the full extent of her connections to Kang Byung-joo, the corrupt police commissioner who founded the hospital and whom Hyung-sik served. The documentary-style unraveling of documents and testimonies suggests Byung-joo manipulated Soo-Ah’s mother into surrendering assets and entangled everyone around her in a bid to tie up loose ends. The public response becomes a catalyst for truth, and Soo-Ah’s conscience begins to surface.

As the case moves toward closure, Nam-Soo presents his findings to the public, and Soo-Ah steps forward to acknowledge her ties to Byung-joo. The film tilts toward a final reckoning: Byung-joo is implicated in the machinery behind the hospital’s corruption and his eventual fate is sealed by his own desperate attempts to avoid a life sentence. Soo-Ah is vindicated in court, but the revelations still threaten to unleash more secrets than they resolve.

In a dramatic twist of memory and revelation, Soo-Ah discloses a hidden truth to Nam-Soo: the diary he possesses actually belongs to Dong-sik, not Soo-Ah. The rescue that seemed to save one life had, in fact, involved Soo-Ah herself, whose lover Woo-Jin was revealed to be her late father. The person killed by Hyung-sik that fateful day was Soo-Ah’s mother, not Soo-Ah herself. The final revelation reveals that Soo-Ah was the vessel of revenge: she is the one who killed Byung-joo. In a strange, compassionate coda, Nam-Soo chooses to bury the most painful details, allowing Soo-Ah to pursue a semblance of a normal life—free, at last, from the nightmarish web of lies and violence that once surrounded her.

The film blends procedural intrigue with a mournful meditation on power, memory, and responsibility.
