On a secluded island, Hwa Tam Kon-Chung and his girlfriend are killed by a chainsaw-wielding man wrapped in bandages, an opening that casts a grim shadow over the parent story. The event sets a chilling tone for what follows as the island becomes a stage for a tense, murderous game.

Three couples arrive by boat to set up a three-day camping trip, a scenario that adds a layer of normalcy before chaos erupts. The line-up features Professor Jazz Lam and Linda Ling Ling Chui, Ken Benny Lai Chun and Winnie Winnie Leung Man-Yee, and Soldier with Be Be Pui Wan-Chak. They intend to enjoy the island’s isolation, a plan thwarted by the arrival of danger in the form of a ruthless band of smugglers led by Boar [Anthony Wong].

Boar’s group presents themselves as something innocuous—a prophylactic salesman on the surface— but their true motive is far more sinister as they search for Hwa. Boar’s henchmen, Pervert Chan Nam-Wing and Blowie Samuel Leung Cheuk-Moon, roam the area and stumble into trouble, including an incident where a deformed boy is unintentionally harmed during a confrontation that foreshadows the brutality to come.

As tensions rise, Pervert is captured by the boy and his father, who dismembers him with a chainsaw, a brutal act that signals the savagery that lurks on the island. Blowie voices concern to Boar about Pervert’s fate, but his warning is dismissed, pushing the smugglers deeper into their dangerous path. The bandaged man—whose home and motives become central to the nightmare—seizes the opportunity, abducting Boar and Mi Mi [Yeung Fan] from the camp and bringing them to his house.

In a misguided attempt to secure his freedom, Boar tries to force Mi Mi into a perverse arrangement with the bandaged man’s son, a moment that erupts into violence when Mi Mi resists and the man kills her. Boar escapes but triggers a tripwire, meeting a grisly end as punji sticks spearlike his path.

Back at the camp, Linda and Be Be discover Boar’s body, and Be Be is captured by the bandaged man. The aftermath is traced by Linda’s warning to the others, and Soldier ventures out to find Be Be, only to stumble into the killer’s lair where he meets his end with his own knife.

Blowie meets a grim fate next, pursuing a lost cell phone he and Pervert had earlier misplaced, and the killer resumes the hunt, eventually decapitating Be Be when she rejects his son’s advances. A fierce fight erupts among the remaining campers, separating them and leading to the capture of Winnie and Linda.

Professor [Jazz Lam] and Ken [Benny Lai Chun] uncover the killer’s home by tracing his oblivious son, and they manage to rescue the two girls by taking the son hostage. In the ensuing escape, the professor triggers the reset punji sticks, resulting in the deaths of both the professor and the son, a cruel pivot in the island’s deadly cycle.

The following day, Ken and the two sisters set a trap for the killer, baiting him into a snare that yanks him into tent spikes embedded in a tree. It appears the family’s immediate threat has been neutralized, and a grim sense of victory briefly settles over the camp.

Yet the killer’s unseen wife emerges, cutting Linda’s neck with the chainsaw that the husband had dropped, ramping up the danger in a final shock. She chases Ken to a cliff, and he manages to topple her from the edge, ending her threat.

With the deranged family seemingly dead, Ken and Winnie return to their campsite just as a boat arrives to take them away, offering a last glimpse of fragile relief. But the sea remembers, and as the boat pulls away, the killer’s wife’s hand suddenly surges from the water, grabbing the side of the vessel in a chilling, unresolved nod to the ongoing menace that shadows the island.
