John “Breacher” Wharton, Arnold Schwarzenegger, leads the DEA’s Special Operations Team (DEA SOT), a tightly knit unit that fights the cartel with precision and grit. The group includes James “Monster” Murray, Terrence Howard, a veteran operative whose loyalty runs deep; Monster’s wife Lizzy Murray, Olivia Williams, whose presence adds a volatile edge to the team; Joe “Grinder” Philips, Sam Worthington, a steadying force with a willingness to take big risks; Julius “Sugar” Edmonds, Eddie “Neck” Jordan, Tom “Pyro” Roberts, Bryce “Tripod” McNeely, and “Smoke” Jennings, all bound by a code of brothers-in-arms and the weight of choices made under pressure.

During a raid on a cartel stronghold, Smoke is killed as the team secures a critical blow: they steal $10 million from a hidden cash stockpile and hide it in the sewer beneath the building, while blowing up the rest and depriving the cartel of the remaining $190 million. The money vanishes, and their superior Floyd Demel suspends them while the DEA probes the discrepancy. The investigation grows tangled with rumors of a parallel FBI inquiry into the cartel, and after six tense months with no confessions or evidence, the team is reinstated to continue the fight.

The case takes a grim turn when Pyro awakens from a drunken stupor to find his motorhome towed onto a railroad crossing, where a train wreck claims his life before he can escape. Atlanta Police homicide detective Caroline Brentwood and her partner Darius Jackson are assigned to the case and begin quietly connecting the dots that point toward the missing money—and toward the cartel’s tangled web of retaliation.

Brentwood quickly sees Neck’s resistance to interrogation and, with Breacher, visits Neck’s home hoping for cooperation. They uncover Neck’s brutal end—nailed to the ceiling and disemboweled—a clear cartel signature that reinforces Breacher’s fear that the cartel is hunting his team over the stolen funds. The danger intensifies as they later visit Tripod, who has left the DEA in a swirl of suspicion and violence, only to be found dead after a firefight in which an assailant is killed—the cartel enforcer identified by Breacher as a target, a hint that the violence is suddenly personal and escalating.

Monster later pays a visit to Brentwood, who remains wary of Breacher’s motives. Monster reveals a painful truth: Breacher’s family was abducted by the cartel, their murders videotaped and sent to Breacher with his wife and son’s severed body parts. Months of searching for revenge and closure follow, until Brentwood’s compassion leads to a moment of intimacy between them.

As the investigation unfolds, Jackson traces a dead enforcer’s cellphone to a cartel safe house and Breacher leads a raid, only to find the enforcers dead at the bottom of a river near Tripod’s home. Brentwood deduces that the cartel did not kill the DEA team; someone inside is framing the cartel for their murders. Breacher then gathers the team to reveal a hard truth: Grinder’s confession to stealing the money is followed by Lizzy killing Grinder, and Lizzy and Sugar orchestrate the killings of their teammates to cover their tracks.

The confession triggers a ferocious unraveling: Lizzy shoots Grinder from a distance, and in a brutal sequence, Lizzy’s true motives come to light as she pushes for escape with Sugar. Monster’s body is discovered stuffed in a refrigerator, raising the stakes as Lizzy calls Breacher—posing as someone who is alone—setting up a final confrontation in a parking garage. Brentwood hides in a car as Breacher and Lizzy collide in a tense pursuit; Lizzy and Sugar try to flee, but a devastating crash into a tow truck decapitates Sugar, leaving Lizzy to face Breacher.

In the confrontation that follows, Lizzy accuses Breacher of stealing the money and seeks vengeance, while Breacher reveals the truth: he took the money for a purpose beyond mere survival. In a decisive moment, Breacher kills Lizzy as police arrive, and he vanishes into the night, leaving Brentwood frustrated and with more questions than answers.

Weeks later, Breacher has traveled to Mexico, where he uses the stolen money to bribe a corrupt police official who can help him identify Brujo, the man responsible for Breacher’s family’s murder. He tracks Brujo to a Mexican bar, where he executes Brujo and the loyalists who stand with him. Wounded, Breacher sits at a table, takes a final shot of whiskey, lights a cigarette, and allows himself a quiet moment of grim satisfaction as he faces an uncertain fate.
