What Happens in Intruder: Full Plot Summary

The whole story of Intruder (2020) explained, from the opening scene to the final frame: every key event, every twist and what it all adds up to. Read it as a quick recap before the sequel, to check a scene you missed, or to finally understand what really happened.

Kim Moo-yul plays Kang Seo-jin, an architect who lost his wife in a hit-and-run about six months ago, and who struggles to manage his work while taking care of his daughter Ye-na.

Seo-jin soon receives a call from an orphanage claiming his long-lost sister has resurfaced after 25 years, Kang Yoo-jin, Song Ji-hyo. He meets her, but neither of them has clear memories about the day of her abduction, leaving a fog of questions hovering over the reunion. The pair visit the hospital where Yoo-jin works, and there they encounter Yeon-joo, a nurse, Han Hye-ji, who praises Yoo-jin and swears by her kind demeanor.

Yoo-jin quickly wins over Seo-jin’s family, and his parents and daughter begin behaving differently, suddenly listening to Yoo-jin and giving her the authority in the house. This swift loyalty makes Seo-jin increasingly suspicious, and he wonders if Yoo-jin could really be his sister after all. The police deliver news of new evidence in his wife’s accident, and Seo-jin spots Yoo-jin in the CCTV footage, which only deepens his unease. In a bid to unravel the truth, he turns to hypnosis therapy through a psychiatrist friend and finds himself face-to-face with the driver who hit his wife—and with his supposed sister—watching from a distance.

Desperate for answers, Seo-jin traces the orphanage but discovers it does not exist. He uncovers a startling truth: Yeon-joo was hired by Yoo-jin to tell the family about Yoo-jin’s nursing role and to cultivate the illusion of a normal family unit. Enraged, he confronts his family with the revelation. Yoo-jin apologizes, explaining she wanted to make a good impression, but the moment is interrupted when their mother faints and is taken to the hospital. Seo-jin manages to secure a strand of Yoo-jin’s hair to submit for DNA analysis.

One night, a realtor phones with news of someone lurking in Seo-jin’s old home. When he arrives, he finds Yeon-joo’s dead body and collapses. He awakens to find himself tied up by the man who killed his wife, who reveals that he led a cult of believers who worship The Chosen Holy Child and abduct girls for its rituals. The current “chosen” child is Ye-na. Seo-jin kills the man and returns home, where police and his friend insist he’s acting erratically because of his medication, and he is arrested.

Yoo-jin confesses that she had met Seo-jin’s wife and coerced her into joining the cult. Seo-jin then flees as Yoo-jin tries to escape with Ye-na, leading to a tense struggle that ends with Yoo-jin nearly going over a cliff. She pleads with Seo-jin not to let go, but he remembers the day of his sister’s abduction and lets her fall, declaring that even if she were truly his sister, she could never be part of their family. In the final moment, Seo-jin shreds the DNA analysis he had ordered, choosing not to look at the result and leaving the truth unresolved.

Q1

Why does Seo-jin suspect Yoo-jin isn't his real sister?

Seo-jin grows suspicious because Yoo-jin wins over his family too quickly, causing them to listen to her instead of him. He also spots her in CCTV footage related to his wife's hit-and-run accident. Additionally, when he visits the orphanage to verify her story, he discovers it doesn't exist, and he learns that Yeon-joo was hired by Yoo-jin to create a false narrative about her nursing background.

Q2

What is the cult in Intruder and what do they want with Ye-na?

The cult worships something called The Chosen Holy Child and abducts girls for ritual purposes. The killer reveals that Ye-na is the current chosen child. This explains why the cult targeted Seo-jin's family - they needed access to his daughter for their religious ceremonies.

  • cult
  • chosen holy child
  • Ye-na abduction
  • ritual
Q3

How did Yoo-jin manipulate Seo-jin's family?

Foreshadowing: Yeon-joo praises Yoo-jin at the hospital, appearing genuine but later revealed as paid acting.

  • Yoo-jin manipulation
  • Yeon-joo hired
  • family deception
Q4

Why does Seo-jin let Yoo-jin fall at the end?

During the cliffside struggle, Seo-jin recalls the traumatic day when his sister was abducted 25 years ago. Even when Yoo-jin pleads for her life, he chooses to let her fall, stating that even if she were truly his sister, she could never be part of their family after all the harm she caused.

Q5

What happened to Seo-jin's real sister?

Seo-jin's real sister, Kang Yoo-jin, was abducted 25 years ago when they were children. The film never explicitly reveals her fate - she remains missing throughout the story, and the impostor exploited this tragedy. • real sister
  • abduction 25 years
  • missing sister
Q6

How was Seo-jin's wife connected to the cult?

  • wife cult
  • mom coerced

Q7

Why does Seo-jin destroy the DNA test results?

Seo-jin shreds the DNA analysis he ordered because he chooses not to know whether Yoo-jin was truly his long-lost sister. After letting her fall and deciding family isn't about blood, he prioritizes closure over truth - he would rather live with the uncertainty than face a result that might complicate his grief.

Foreshadowing: This decision echoes his earlier choice to let Yoo-jin fall rather than save someone who betrayed his family.

  • DNA test shredded
  • truth vs closure
  • unresolved ending
Q8

Who killed Yeon-joo and why?

Yeon-joo is killed by the same man who killed Seo-jin's wife - a cult member who was eliminating loose ends. After Seo-jin discovers her body at his old home, the man captures him and reveals the cult's existence. Yeon-joo was killed because she knew too much about Yoo-jin's deception and was no longer useful to the cult's plan.

  • witness Yoo-jin
Q10

Why does Yoo-jin pretend to be Seo-jin's sister?

Yoo-jin infiltrated Seo-jin's family as his sister to get close to Ye-na, who was chosen as the cult's Holy Child. By becoming part of the household, she could facilitate the girl's abduction for their ritual. The impostor scheme was a calculated move to use family access for the cult's purposes.
