[REC] 4: Apocalypse

A Special Forces Team is sent to an apartment complex to rescue Ángela and destroy the building. They fight off infected firefighters and residents as the mission unfolds. Lucas and Guzman survive the operation and complete the assignment.

Ángela awakens chained to a bed on a quarantined vessel with memory gaps about what happened. Dr. Ginard orders blood tests while the crew tries to understand the situation from within the isolation.

Lucas and Guzman are on the same quarantined ship, joining Ángela as they try to figure out who is infected and what the next steps will be. The vessel’s isolation keeps everyone cut off from outside help. They brace for the unknown threat ahead.

Ángela makes a second escape attempt but fails to break free. Dr. Ricarte then informs her that she is not infected, giving her a momentary sense of relief amid the tension onboard.

Guzman befriends the ship's captain Ortega and technician Nic. Using Ángela's camera, Nic begins reconstructing the events inside the apartment complex to inform the doctors about what happened.

The doctors discover that a monkey infected by the pathogen was released during a power outage. The animal infects the ship's cook, who contaminates food and triggers a wider spread among the crew.

Ángela, Guzman, Lucas, and a surviving elderly woman try to reach the bridge. Along the way, the elderly woman is separated; Lucas stays behind to rescue her and discovers she is infected, forcing him to shoot her.

Ricarte realizes that the antidote cannot be found because it would require an unmutated strain of the pathogen. The reality of the outbreak becomes more troubling as the investigators uncover deeper infection dynamics.

Nic sends the reconstructed film to the doctors, and it shows Ángela being orally infected by a worm-like parasite carried by the pathogen. This revelation reframes the infection as a parasitic transmission rather than a straightforward contamination.

Ricarte attempts to surgically extract the parasite from Ángela, but an attack by the now-infected Ginard disrupts the procedure and allows Ángela to escape. The tension on the ship escalates as infection spreads and the medical team scrambles for a cure.

Ángela ambushes Ricarte and bites him, intensifying the suspicion that he has been exposed. Despite a blood scan showing negative results, Ricarte remains convinced Ángela is deceiving him about the infection.

It is revealed that during Ángela's rescue, the parasite has transferred to Guzman. Guzman traps Ángela in the ship's bowels as Ricarte initiates a self-destruct sequence.

Lucas and Nic attempt to start the ship's auxiliary engine but fail, and Lucas is killed by the infected. Meanwhile, the crew contends with dwindling options as the ship's fate grows clearer.

Nic finds Ricarte attempting to flee in a lifeboat and knocks him out, taking the lifeboat for himself. He then searches for Ángela, determined to survive and reunite with her.

Nic and Ángela fight off the infected monkeys and confront Guzman, killing him with a harpoon to secure their escape. They flee the wrecked ship and dive into the sea as the vessel explodes behind them. In the underwater aftermath, the parasite twisting is swallowed by a fish, hinting at a possible continuation.
