A woman captures joyful moments of her daughter and husband on film. Shortly after, we see this same woman in a cramped room as she makes a futile attempt to reach her daughter, Megan, whose call goes unanswered. This woman is revealed to be Dr. Lauren Chase, as indicated by her badge.

Lauren is soon briefed by Doctor Greer, a military officer, about her urgent role with an extraction team. Her responsibility is to conduct thorough tests to prevent any infected individuals from being brought back to the military base. During this briefing, Lauren is introduced to the different levels of a mysterious infection, with level 5 displaying extreme aggression and danger.

As Lauren joins her team comprising of Wheeler, the driver, Denise, the navigator, and Gunner, they are tasked with locating a group of uninfected individuals who are barricaded inside a school. Denise informs Lauren about Gunner's wife, who was part of the previous team that discovered the survivors but has since gone missing. The journey takes a grim turn when they stumble upon the bus used by Gunner's wife’s team, and he is devastated to find her half-eaten corpse inside.

Upon arriving at the school, Lauren tries reaching out to her daughter again, and this time Megan answers, revealing that she is at their home in the Los Angeles suburbs. Lauren promises to come and get her. However, their exploration of the school takes a terrifying turn when they discover that the purported survivors have now been infected. In a desperate act of courage, Gunner sacrifices himself, allowing Lauren and Denise to escape. In the chaos, Wheeler abandons them, fleeing to save himself.

After finding temporary refuge, Lauren and Denise connect over their shared experiences as mothers. As they discuss Lauren's conversation with her daughter, Lauren comes clean about her identity—the woman they believe to be Dr. Lauren Chase is actually Rebecca Thomas, who had stolen the badge. Denise pledges to help Rebecca locate her daughter. Their safety is soon threatened again when a level 5 infected creature attacks, fortunately, they are rescued by Wheeler, who reveals he found another doctor in a nearby hospital. However, the doctor is skeptical of Wheeler’s story and presses Rebecca to convince him of her credentials.

The journey to the hospital is fraught with danger, culminating in an attack by a group of infected individuals that results in Denise being captured. Under a bridge, the infected brutally butcher her, but in an act of fury and desperation, Rebecca fights back, killing one of them before Wheeler brings her back to her senses. At the hospital, they find the doctor distrustful of Rebecca's story, having met the real Lauren Chase before. He forces them out, leaving them to find another escape plan.

With Wheeler’s resourcefulness, they acquire another vehicle and head toward the suburbs with hopes of finding Rebecca's daughter. Upon their arrival, Rebecca is heartbroken to discover Megan has been infected but is still alive. Tragically, she also finds her husband’s corpse in the garage. Outside, Wheeler is attacked by infected individuals. In a desperate moment, Rebecca instructs her daughter to wear her protective suit. They manage a narrow escape through a window; however, Wheeler suffers severe injuries, urging Rebecca to continue without him. As Rebecca and Megan approach the compound, chaos reigns as infected individuals attempt to breach the perimeter. In a misidentification moment, the military perceives Rebecca’s actions as hostile and opens fire, mistaking her for a threat. Despite Megan showing clear signs of infection, Doctor Greer, believing Rebecca to be Lauren Chase, orders the soldiers to take the two inside for medical treatment, leaving their fate uncertain amidst the harrowing crisis.
