Initially, as the opening credits roll, we witness a military force, equipped with guns, flame throwers, and dressed in Bio Hazard suits, entering a dark building. The scene is accompanied by a chilling emergency call to 911 in which a female operator communicates in Spanish, while a male voice on the other end speaks in English, revealing the dire situation: > “They are sick, they are all sick. There is blood, there is blood everywhere… they’re looking for me.” Upon entering, the armed force discovers a gruesome scene filled with multiple dead bodies, grotesquely deformed and in a state of advanced decay. Amidst the carnage, they find a man named Porter, who is taken back by force to a medical research facility located on an island off the coast of The Dominican Republic.
As the story unfolds, we encounter Dr. Edwards and his assistant Camila walking through the laboratory corridor. During their discussion, Camila mentions the isolated incidents in the States and expresses confusion about how the virus reached the D.R., hinting at the events following the earlier Cabin Fever films. It turns out Porter is Patient Zero, a carrier who remains immune to the virus. Despite the tragic knowledge that his son was one of the virus’s early casualties, Dr. Edwards sees an opportunity to use Porter’s immunity as a potential catalyst for creating a vaccine or a cure, pushing him to assist against his will.
In a parallel storyline, we meet Marcus as he drives through the Dominican countryside to meet his best friend Dobbs. They’re on their way to a rehearsal dinner for Marcus’s impending marriage to Katia. The film playfully shifts to the dinner, where characters like Josh and Penny engage in their own antics. As the new couple celebrates, Marcus, Josh, Dobbs, and Penny embark on a rented yacht to a supposedly deserted island for one last adventure before the wedding, despite Katia’s warning about not sailing too far north due to restricted waters.
Back at the medical facility, Porter expresses his distress over being forced into research against his will. His dialogue with Camila reveals his desperation to contact his wife and plead for help: “You can’t just keep experimenting on me like this.” A cold response from Dr. Edwards, who refers to Porter as “The Specimen,” highlights the moral conflict brewing beneath the surface. Amid hospital chaos, Porter ingeniously fashions a weapon from a metal spring, leading to a brutal confrontation that triggers a violent series of events, culminating in a security lockdown that traps him and several others in the lab.
Meanwhile, on the yacht, the atmosphere turns from festive to sinister as Penny begins to display concerning symptoms. Their snorkeling expedition leads them to a graveyard of decaying fish, a harbinger of the malignancy that has begun to spread among the group. As illness takes hold, tension escalates, and relationships strain under the pressure of terror.
The narrative spirals into chaos as Dr. Edwards continues his grim research amidst a lockdown. The juxtaposition of the partygoers’ revelry on the yacht with the increasingly horrifying events at the facility reinforces the duality of life and death in this thriller. As Marcus and Dobbs infiltrate the facility in search of help, they uncover a horrifying truth that ties back to Porter’s actions and motivation.
