Craig Mannings is a respected doctor in the United States who begins to experience strange visions of women being murdered. He discovers these visions may stem from a psychic link with his twin brother Keith Mannings, who is believed to have died in a Cleveland house fire at 17 but is now implicated in a chilling murder spree. To understand and stop his brother, Craig travels to Hamburg, Germany, despite protests from his girlfriend Julie Warren.
In Hamburg, the chase intensifies when Keith is spotted and mistaken for Craig at a busy coffee shop by ex-boxer Bud Waldo, one of Craig’s former patients. Keith goads the older man into an impromptu boxing match, delivering a brutal blow that triggers Waldo’s fatal heart attack. Craig arrives and meets Waldo’s daughter Christine Waldo, who quickly joins his search for Keith. The pair grow closer, but their efforts are hampered by the local police, led by Inspector Hessinger.
As Keith closes in, he locates them, rapes and kills Christine, and confronts his brother, explaining that their psychic link let him know how to draw Craig to him and that he committed the murders to lure his brother closer. Craig condemns the violence, but Keith escapes, promising more murders. Soon after, the police arrive and arrest Craig, charging him with Christine’s murder.
Julie Warren arrives in Germany, certain that Craig’s visions are real, and urges the authorities to search for Keith in connection with the murders that Craig now faces. Craig is exonerated after Keith commits two more killings—a prostitute who had been a regular sight and then a cab driver. While Craig remains in custody, Julie devises a plan to use herself as bait for Keith at a secret location, counting on the psychic link to bring the police to her in time. Keith, unstable and obsessed with his brother, rapes Julie and tries to strangle her, but she fights back and kills him with his own knife.
Craig experiences a final vision in which Keith killed their parents years earlier. In another vision, Keith’s body appears to possess Craig, locking him in a morgue drawer. Keith then appears to Julie again, inhabiting Craig’s body, and she realizes the terrifying truth as he kisses her.
