Read the complete plot breakdown of The Twin (2022), including all key story events, major twists, and the ending explained in detail. Discover what really happened—and what it all means.

Rachel, along with her husband Anthony and their son Elliot, relocates from New York to Finland, grappling with the profound grief of losing Elliot’s twin brother, Nathan, in a tragic car accident. During an outing, Elliot stumbles upon a mysterious wall said to have the power to grant wishes and makes an undisclosed one. Soon after, he begins to converse with an unseen entity, concerning Rachel deeply.

At a welcome gathering orchestrated by the local residents, Rachel encounters the town doctor and a woman named Helen, who cryptically alludes to Elliot’s secret wish. As time passes, Elliot’s behavior becomes increasingly unsettling, startlingly announcing that he is actually Nathan. When Rachel seeks help from the town doctor for Elliot, he shockingly claims that she herself needs psychiatric care, dismissing her concerns. In her desperation for clarity, Rachel consults Helen, who divulges that the town is steeped in dark rituals associated with a satanic cult. Helen further explains that her deceased husband had been possessed by a malevolent force which was evident in photographs.

Frightened and confused, Rachel snaps pictures of Elliot only to discover, later on, that he is absent from the images when developed. Alarmed, she presents the photos to Helen, who interprets this as a torment from a demonic presence. Helen persuades Rachel to return with her to Elliot in a bid to save him, but upon arrival, she instead informs Rachel that she is the one unwell. Soon, they find themselves encircled by the townsfolk, where Rachel is sedated and coerced into participating in a satanic ritual. Upon regaining consciousness, she finds herself imprisoned in her own room but manages to escape upon hearing Elliot’s cries for help.

As Rachel flees into the forest, Anthony reveals the shocking truth: Elliot is a figment of her imagination, created out of her grief and anguish after the accident where she intended to leave with Nathan. To protect Rachel from being institutionalized, Anthony had played along with her delusion, moving them to Finland for a fresh start. Tension mounts as “Elliot” makes a desperate escape, claiming that Anthony seeks to harm him. In a gripping confrontation at a grain silo, Anthony attempts to persuade Rachel by submerging Elliot in grain. The situation culminates tragically when Rachel inadvertently kills Anthony while trying to rescue her son.

Ultimately, Rachel fails to find Elliot amidst the grain, and the film concludes with her returning to New York, still ensnared in her own delusions. Despite visiting the graves of Nathan and Anthony, she maintains the belief that all three, including Elliot, are somehow still alive, trapped in her fragmented reality.
