In a chilling prologue, a young girl catches sight of an unfamiliar car parked outside her home through her bedroom window. Curious, she grabs her Polaroid camera and ventures outside, only to find a cloaked figure in the passenger seat. Just as she is about to investigate further, she hears a voice calling from behind her house, leading her to a person with a hauntingly pale face.
Fast forward, FBI agent Lee Harker is embroiled in a frantic manhunt for a serial killer. While canvassing a neighborhood, her partner is tragically shot and killed. Relying on sheer instinct, Harker manages to apprehend the suspect. Subsequent evaluations reveal her to have psychic abilities, and she is subsequently assigned to the case involving the elusive Longlegs — the main suspect behind a series of heinous murder-suicides targeting families. The disturbing trend shows that all victims share the same birthday, with each crime scene featuring a cryptic letter signed by “Longlegs” and devoid of other forensic evidence. Harker’s daunting task involves deciphering these letters, replete with satanic symbols and secret codes.
After a grueling first day, Harker takes her supervisor, Agent Carter, home. There, she meets his wife Anna and their daughter Ruby, reluctantly agreeing to attend Ruby’s birthday party in a week’s time. That evening, a knock at her door sends chills down her spine as she steps outside to find a shadowy figure — only to return and discover a letter from Longlegs addressed directly to her, along with a disturbing birthday card.
Delving deeper, Harker follows a clue marked “X marks the spot,” discovering a cross on the floor of an old crime scene. Upon investigation, they unearth a doll buried beneath it. Examining the doll reveals a strange metallic orb hidden inside its head. An ultrasound on the orb uncovers unsettling whispers and triggers Harker’s psychic visions.
In an effort to connect the dots, Harker visits Carrie Anne Camera, the lone survivor from one of the previous crime scenes, currently residing in a mental institution. Carrie Anne recounts her haunting experience of receiving a doll from Longlegs for her birthday—a gift that preluded her father’s horrific murder of their family. The girl describes hearing disembodied voices from the doll, feeling compelled to obey them at any cost.
Convinced that Longlegs has an accomplice, Harker analyzes the murder dates, recognizing they form an upside-down pentagram, leaving one date conspicuously missing to complete the pattern. After a visit to her mother’s house yields a treasure trove of Polaroids from her childhood, Harker spots a picture of the pale-faced man she believes embodies Longlegs. This crucial find propels them to identify and apprehend him in broad daylight as he waits for a bus.
During an intense interrogation session, Longlegs reveals his true identity as Dale Ferdinand Kobble, whimsically singing “Happy Birthday” to Harker. When pushed about his motives, he chillingly claims allegiance to “the man downstairs,” a reference to Satan himself. When asked about his accomplices, he ominously hints that she should “ask [her] mommy” and then proclaims, “Hail Satan!” before brutally smashing his head against the table to commit suicide.
The plot thickens as it is revealed that Harker was the little girl who encountered Longlegs all those years ago. Seeking answers, she drives back to confront her mother, only to witness a traumatic scene: her mother fatally shoots her colleague with a shotgun. Emerging outside to find her mother, Harker is confronted with a doll resembling herself that her mother is poised to shoot. The moment results in Harker fainting from shock.
A montage elucidates the haunting truth: Harker’s mother was, in fact, Longlegs’ accomplice, masquerading as a nun who distributed the cursed dolls to unsuspecting families before the murders. Longlegs had been lurking in the family’s basement, a demonic presence known as “the man downstairs,” who made a sinister pact to spare Harker’s life during her childhood. As for Harker’s doll, it remained intact and fortified her psychic abilities.
Waking up groggily in the basement to the sound of a ringing phone, Harker rushes upstairs to answer it. The deep voice on the line chillingly asserts, “You’re late for Ruby’s party.” With urgency driving her, Harker commandeers Longlegs’ car and races to intervene. She arrives to find her mother already at the Carter household, where the entire family appears entranced by the sinister doll. As her partner Carter unexpectedly murders his wife in the kitchen, Harker is forced to act, taking aim and killing Carter to safeguard Ruby.
While in an intense confrontation with her mother, who remains intent on sacrificing Ruby to complete the ritual, Harker is left with no choice but to shoot her own mother, breaking the eerie trance to halt the killings. However, the struggle intensifies as she realizes she has run out of bullets just as she faces the doll—her last chance to eradicate the source of evil.
The film wraps up with a haunting image of Longlegs in the interrogation room, once more defiantly uttering “Hail Satan” and blowing a mocking kiss to the audience, leaving a lingering sense of dread and unresolved terror.
