We open on the death of Gary Dean West and Mervo Tiny Tim’s parents, a scene that suggests murder before the bodies are carted away by what look like paramedics. In the wake of this tragedy, Jill Itonia Salchek Robinson returns to her small hometown, fresh from college, only to discover that the townspeople harbor resentment toward her father, a banker who has been foreclosing on local farms. At the diner, where her friend Sarah Lori Minnetti works, Jill reconnects with old ties and accepts a ride home, but home itself holds bad omens: her parents are missing, the house has been vandalized, and a bizarre riddle has emerged in the form of Mervo, dressed as a clown, roaming the kitchen as part of his new identity.

In the midst of this unsettling scene, the house is also occupied by Gary, the childhood neighbor and Merv’s brother and caretaker. Gary Dean West arrives and takes Mervo away, leaving Jill to piece together the strange dynamics of the family she once knew. As Jill tries to make sense of the situation, she seeks help from the Sheriff Frank Benson, hoping that official courage can cut through the fear that has gripped the town. Yet the Sheriff’s inspection seems to reveal more than it proves: the windows, the paint, and the whole house appear fixed by some unseen, unseen hand, prompting Jill to recall their shared childhood in the old treehouse and to reexamine the bonds that tied them together.

As the memories begin to flood back, Jill confesses that she is engaged to a man named Scott Peter Krause. This revelation stings Gary, whose own past and present intertwine with the mystery surrounding Jill’s family. Gary also reveals that his parents were murdered, a truth that lends dark significance to Merv’s clownish persona, the apparent coping mechanism for the family’s trauma. The tension intensifies when Mervo is seen bringing food to an unseen woman who is being held hostage, a detail that deepens the sense of danger enveloping Jill’s world.

That night, the pressure culminates in a nightmare of stalking and intrusion. Jill endures harassment from a masked figure, with possible involvement by Merv/Mervo, and she again calls the Sheriff for protection. When the Sheriff and Jill return to the house, they find nothing amiss, as if the threat has shifted or become more elusive. Gary recedes into the background for a moment, only to reappear and admit that he fixed the windows and repainted the house, attempting to steady the situation as memories of childhood resurface. The tension between Jill and Gary grows more intimate as they reminisce and confront the truth about their tangled past.

The next day brings another wave of terror. Jill’s fiancé Scott arrives with expectations that stray far from comfort, and his attention quickly becomes aggressive. In a disturbing turn, Scott has sex with Jill while keeping his jeans on, an act that Gary spies on from outside the window, heightening the sense of surveillance and danger. Scott leaves to fetch beer, and as he follows a suspicious figure into the barn outside, he is attacked. Gary circles the house to speak with Jill, but his attempt is interrupted by Merv/Mervo, who forces him to depart with him. Sarah briefly appears another time, only to be seized and attacked by the masked intruder outside, and she is taken to the barn, where she is killed.

When Gary returns, Jill discovers that Sarah never made it to work and that Scott has not returned either, prompting the heartbreaking conclusion that the two may have fled together. Believing they have eloped, Jill grows desolate, only to be drugged again later that night by the masked figure. The intruder’s plan to shepherd Jill to the barn is interrupted by the Sheriff’s arrival, which ends with Scott tied up but alive and his throat later slashed by the masked figure.

Morning reveals a shocking symbol of decay: Jill finds a bucket of blood in her fridge. Gary, somehow already in the house, comes to her side, helps her upstairs, and then undresses and bathes her before carrying her back to the couch. While Jill sleeps, Gary returns to her side with a dangerous hunger, undresses, and climbs on top, an unsettling moment that Jill resists before demanding he leave. The unsettling visitor, Mervo/Merv, arrives again and lures Jill to their own house, where the truth is laid bare: their parents’ suicide note, the sensational photos of Jill naked and sleeping, and the revelation that Gary is the masked figure behind many of the crimes.

Gary confronts Jill with his violent claim that loneliness and the murder of everyone she loves will bind them forever. He drags her toward the barn, intent on finishing what he started, but Merv/Mervo intervenes and shoots Gary to protect Jill. In the aftermath, Merv/Mervo explains that Gary had coerced him into secrecy about the killings, and the two men walk out of the barn together, leaving Jill to grapple with the horror of what she has survived. The film ends on a chilling note, with Gary still alive and opening his eyes, and the fate of the tied-up woman—likely Jill’s mother—left unresolved in the shadows of the barn.
