Lt. Parker Barnes, portrayed by Denzel Washington, steps off a subway car alongside his partner, Donavan, into a distorted world as they navigate a bustling city street. Unsure of their destination, Donavan is confused while Barnes strides confidently toward an impressive Japanese structure. When asked by Donavan what they are searching for, Barnes cryptically responds that they’ll recognize their target by the man who has eyes similar to his own. They soon spot a figure, played by Russell Crowe, concealed behind a screen, leading them to open fire. The mysterious man narrowly evades their shots, retaliating with a firearm of his own, and swiftly knocks out Donavan, vowing to return for Barnes.

Inside the building, the confrontation takes a dangerous turn when the enigmatic foe surprises Barnes and injures his arm. Although Barnes manages to hit him in the arm, the villain resorts to underhanded tactics, leaving Barnes at a disadvantage. The revelation unfolds that they are trapped in a perilous virtual reality program, leading to Donavan’s tragic demise. Behind the scenes, Wallace and Elizabeth Deane analyze the situation, debating their safety measures as they drive away. Barnes insists that the adversary, known as SID 6.7, has cheated, having accessed abilities like electrocution that weren’t supposed to be available. Meanwhile, police chief William Cochran assures Barnes he is working on his release, but all Barnes truly desires is chalk for his cell, leading to a reprimand when he shoots a simulated figure.

After returning to his automated prison cell, it becomes evident that Barnes has suffered severe injuries, including a metal arm. As he walks through the hall filled with derisive crowds, he is attacked by another inmate but manages to triumph with impressive ease, leading to his apprehension by guards. Meanwhile, Daryl Lidenmeyer, the program’s creator, confronts SID, who brazenly boasts about his taste for violence, indicating that Lidenmeyer must sacrifice something precious for the continuation of their project.

In a chilling turn, therapist Madison Carter, portrayed by Lynch, enters to evaluate Barnes’s psychology. He claims his actions were defensive, mocking her in the process. During their discussion, Carter reveals her research on SID and probes into Barnes’s traumatic past concerning Matthew Grimes, the man responsible for the death of his family. As the narrative develops, tension escalates when SID, with dark intent, begins carrying out murders styled after historical events, leading Barnes down a path of personal vengeance.

Amid a web of manipulations and hostage situations, SID’s escalating carnage is documented live, all the while taunting Barnes, who is determined to clear his name. In various gripping scenes of pursuit and violence, Barnes confronts the twisted avenger within the confines of both reality and virtual dimensions, leading to a climactic showdown where Barnes must make desperate moves to save Madison’s daughter, Karin.

Ultimately, in a heart-pounding conclusion filled with cunning and bravery, Barnes navigates a maze of booby traps to prevent a catastrophic explosion, reclaiming his agency and enacting justice not just for himself, but for those he has lost. The tension builds as he battles not just an enemy, but the haunting ghosts of his past, striving for redemption and the chance to protect the future.
