Amy, an elementary school teacher, is plagued by nightmares about doctors and hospitals. These dreams stem from the trauma of her father's death and haunt her into adulthood.

In the afternoon, Amy and her fiancé Nick are involved in a serious car accident after he proposes. An ambulance carries Nick away to an unknown hospital, leaving Amy shaken and disoriented.

Amy teams up with Lucas, the truck driver who hit them, as they try to locate where Nick and Lucas's sister have been taken. They race to piece together which hospital is involved and who needs help. The search is frantic and uncertain.

Amy begins to hallucinate, seeing disfigured faces everywhere, including her own reflection in a mirror. The visions blur the line between reality and nightmare and heighten her fear.

A little girl from Amy's class, Melissa, claims she can help Amy find Nick and provides the name of St. Rosemary's Hospital. Her tip sets Amy on a path toward the rumored institution.

Amy and Lucas learn that St. Rosemary's was rumored to be a home for devil worshipers. The hospital was destroyed in a fire about 70 years ago, taking with it all the nurses and staff who refused to flee.

Nick experiences his own horrors at St. Rosemary's, including nurses who spray blood on each other and eat another patient. The hallway he traverses seems to repeat itself, heightening the sense of dread.

After another hallucination, Lucas comforts Amy but is revealed to be a demon taunting her about her father's death. The danger shifts from a human foe to a supernatural threat wearing Lucas's guise.

She escapes Lucas and hails a cab to the hospital, determined to reach Nick. On the ride, she endures attacks by nurses and apparitions of dead patients that try to derail her mission.

Inside the hospital, Amy encounters her father's ghost and relives the moment from when she was 12, when he asked her to unplug the machine keeping him alive. The memory exposes a pivotal trauma in her life.

The Lucas demon catches up with Amy, taunting her that she belongs with the ghosts and monsters at St. Rosemary's because she murdered her father. The psychological assault deepens as the hospital's danger intensifies.

She scalds Lucas with hot steam and escapes his grasp, regaining some control over the perilous situation. The escape marks a turning point in her battle for Nick's safety.

Amy reaches the operating room and frees Nick from the operating table, pulling him away from imminent danger. The physical rescue coincides with the film's heightened tension.

The hospital suddenly begins to burn as Amy and Nick flee toward the exit. They exit into a bright, hopeful light, a symbolic escape from the horrors they've endured.

Amy awakens still in the car crash and realizes her experiences were a test. She understands that she is dying, leaving the outcome unresolved and haunting.
