The film begins with television producer Jerry Hartfield elucidating the concept behind Grave Encounters, a paranormal reality show that was helmed by and starred ghost hunter Lance Preston. Despite its intriguing premise, the show was inevitably canceled after only five episodes. Hartfield clarifies that what follows is the raw footage from the sixth and final episode, with only minor edits made for time constraints.

As the footage unfolds, we see the Grave Encounters crew, which includes Lance along with occult specialist Sasha Parker, technical expert Matt White, cameraman T. C. Gibson, and guest psychic Houston Grey. They embark on an investigation of the Collingwood Psychiatric Hospital, a site notorious for its alleged paranormal activity over many years. During their walkthrough of the hospital and its labyrinthine underground tunnels, they are led by a caretaker who divulges the dark history of Collingwood, particularly the unsettling practices of Arthur Friedkin, a physician known for conducting unethical experiments and lobotomies on patients—before meeting a grisly end at the hands of his escaped patients.

Determined to capture evidence of the supernatural, the crew makes the bold decision to lock themselves inside the hospital for the night, pitching camp near the main entrance, which secures itself firmly from the outside. Initially, the night seems uneventful, until suddenly a door fiercely slams shut behind T. C., prompting the team to seek contact with the unseen forces haunting the hospital. However, the paranormal occurrences intensify, growing increasingly aggressive as the night progresses.

With only half an hour remaining before the caretaker is due to unlock the doors, the crew begins to pack up. In a troubling twist, Matt ventures out to retrieve the cameras they had set up earlier, but mysteriously vanishes. The team frantically searches the hospital, but with no sign of Matt and the caretaker yet to arrive, they resort to breaking down the front doors, only to be confronted with yet another corridor, continued darkness outside—a clear indication that time is distorted within the walls of the building.

Their search continues, and amidst the chaos, they encounter a horrifying apparition: a girl with a distorted, demonic face. In a panic, the crew scatters, and unfortunately, Houston gets separated and suffers a brutal fate at the hands of an unseen entity. As they regroup, the remaining crew finds ominous hospital tags with their own names on them. Eventually, they locate Matt in a disheveled state, draped in a hospital gown, his sanity shattered as he eerily declares that their only chance of escape lies in “getting better,” under the influence of the malevolent spirits that haunt the premises.

The terror escalates as the crew is relentlessly pursued by a slew of ghosts. In a heart-pounding moment, T. C. is dragged into a blood-filled bathtub and disappears, while Matt meets a tragic end, plummeting down an elevator shaft. Lance and Sasha are then beset by a terrifying demon without a tongue. In a desperate search for a way out, they delve into the tunnels, where Sasha’s health declines drastically and she vanishes into a mysterious mist during their attempts to rest.

Desperation drives Lance to forge on alone through the tunnels, resorting to the grim survival tactic of consuming rats. Eventually, he stumbles upon Friedkin’s operating room, replete with a pentagram and altar indicative of dark rituals linking black magic to medical practices. As he comes face to face with apparitions of Friedkin and his malevolent nurses, they seize Lance, dragging him onto an operating table, leading to a brief blackout of the camera.

When the footage resumes, we see a lobotomized Lance, who now claims he is “better” and expresses his farewell to the audience as the screen fades to blue.
