One year after Nicole and Jesse vanished, Jesse’s brother Tom, fresh from active duty, joins friends Marilyn and Jared to find them. Their search leads them down an isolated stretch of highway, where a strange rest stop hides a deadly secret, thrusting the group into the same terrifying encounter with the madman who hunted Nicole and Jesse.

The trio stops at a gas station where Tom shows a photo of Jesse and Nicole to the attendant. The attendant says he did see them and warns them to be careful. Jared finds Nicole's horse-riding badge, which links the present danger to the Old Highway.

Porta-potty attack

Jared uses a porta-potty at a construction site while Tom and Marilyn continue toward California. The Driver harasses Jared and rams the porta-potty with the truck, covering him in feces. The attack marks the first direct threat in the new chapter of the story.

Tom arrives amid the chaos and they discover that The Driver's eyes are with the twins, as foretold by Scotty's ghost. They corner The Driver and set the R.V. on fire in an attempt to destroy him, with The Driver vanishing as the blaze spreads. The eyes are now in the hands of the twins, completing the ritual.

Aftermath: the road goes on

During the aftermath, Jared finds his Nicole photo missing from his visor, signaling the danger is not over. The R.V. collides with Jared's car as the pursuit resumes. In a motel, Tom and Marilyn mourn; the yellow truck is heard outside, and in the final moment, The Driver continues to roam the Old Highway.
