On the desolate Old Highway in 1972, a devout RV family encounters The Driver Brionne Davis, stranded beside a yellow truck with no gas. Inside the vehicle, the mother Diane Salinger flirts with him, while Scotty Mikey Post, the physically deformed boy, is pressed into taking a photo with his camera. That night, the Father Michael Childers discovers The Driver in bed with his wife, and the mother aggressively claims it was non-consensual, spurring her husband to attack. In a brutal ritual of “cleansing,” the family drags The Driver outside and tortures him to death, burying him at a rest stop and keeping his eyes as a gruesome trophy. By sunrise, The Driver’s ghost has exacted vengeance on the family, killing them all and driving away in the yellow truck.

Fast forward thirty-five years, and a year after Jesse and Nicole disappear, Tom Hilts Richard Tillman returns home from Iraq with his girlfriend Marilyn Jessie Ward and Nicole’s longtime friend Jared Graham Norris, who has long harbored a crush on Nicole Julie Mond. They press on toward California to find Jesse and Nicole, stopping at a gas station where a wary attendant offers cryptic warnings. Jared finds Nicole’s horse-riding badge, a clue that intensifies the mystery, and the attendant explains he found it on the Old Highway, about a mile up.

As they split up to cover more ground, Jared pulls over at a construction site to use a porta-potty, while Tom and Marilyn press on. The Driver begins to harass Jared, eventually ramming the porta-potty and trashing him with filth. The rest stop becomes a terrifying nexus when The Driver abducts Tom while Marilyn is in the bathroom; Nicole’s ghost materializes in the stall, vanishing as Marilyn exits to tell Tom what she saw. Their world shifts from pursuit to survival as they realize Nicole’s presence is a warning and a clue.

That night, Jared sanitizes himself, changes clothes, and then, in a chilling moment, Nicole’s ghost returns to his backseat. Believing she is truly there, Jared consoles her, and as their bond deepens, Nicole ends up vomiting blood and disappearing. Jared’s car then dies, forcing him to race into the road where a reunion with the R.V. family is staged in a disquieting, eerie way. They offer him a ride to the rest stop, hinting at the fates that befell Jesse and Nicole, and Jared relays everything to Marilyn, who confirms Tom’s disappearance. Determined, the trio heads back to the gas station to seek answers from the attendant who knows more than he let on.

Back at the station, The Driver tightens his grip, torturing Tom on a school bus until Tom frees himself. He discovers Jesse—still alive but trapped in a cage—whom he rescues, only for Jesse to turn and tell him, “You should have saved me,” before vanishing. Tom returns to the scene, armed and ready for combat, hoping to rescue Marilyn and finish the mission he began years ago.

As dawn breaks, Marilyn searches the abandoned RV for the eyes, aided by Jared who stumbles upon Scotty’s ghost—an eerie messenger who reveals that the twins hold The Driver’s eyes and have since holed themselves away in the RV. In a desperate bid for freedom, Marilyn and Jared destroy the RV by setting it ablaze, driving The Driver back into the darkness as he vanishes once more.

The aftermath reveals a haunting twist: a photo of Nicole that Jared kept on his visor has disappeared, and a fatal collision with Jared’s car seals a grim fate. The tale ends in a motel, where Tom ponders Jared’s disappearance with Marilyn by his side. Marilyn’s tearful confession—“You should have saved me”—echoes through the room as the yellow truck roars outside, and Tom looks toward the cab, where Marilyn’s spectral presence sits beside him.
