During the chase, Preest is seized by local authorities and restrained. He is taken toward a procedure in an operating theater under the promise of release later. The setup hints at a sinister attempt to weaponize him.

Before the procedure can proceed, Preest fights his way out of the building. His escape marks a renewal of his agency and a blow to those seeking to control him. He vanishes into the city, determined to continue the hunt.

In London, Emilia records a dangerous act and takes a deadly amount of pills. An ambulance arrives in time to save her, and she wakes up in a hospital. The incident launches her own psychological journey while the city around her continues to feel unreal.

Peter, a religious man, hunts for his son David who has become unstable after the war. He learns David has been reported at the hospital before disappearing again. The scene contrasts figures of faith against a fragile, haunted family history.

After learning the kidnapped girl has been killed by The Individual, Preest tracks down Wormsnake and attacks him. He pretends to want a talk and hands Wormsnake a note with an address to pass along. He then lies in wait across the street, rifle ready for The Individual.

Emilia's ongoing project involves following and recording strangers, sometimes in disguise. One target is Milo, whose fiancée left him days before their wedding, sparking the belief that a redheaded figure named Sally is everywhere. He plans a restaurant date with 'Sally', unaware of the illusion underpinning his certainty.

Milo discovers from his mother that Sally isn't real, a character he created in childhood to cope with his father's death. Despite this revelation, he still goes to the restaurant for the date. The scene blurs lines between memory, fantasy, and reality.

At a shelter for the homeless, Peter encounters Bill, an old Basra-era acquaintance of David. Bill has been warned David might kill him after David attacked him and left a note with an address. Bill, who resembles Wormsnake, fears for his life.

After a botched suicide attempt and a conversation with the hospital janitor, Emilia confronts her mother about past abuse. Her mother reveals she believed Emilia all along and confirms they left the father. The two women begin reconciling after years of silence.

Peter goes to the address on Bill's note, which turns out to be the building where Emilia lives. He then heads to the restaurant opposite the building, where Milo is supposedly dating 'Sally'. The evening weaves multiple fates together in the same corridor.

David, who looks like Preest, bursts into Emilia's apartment and hits her. He uses the apartment window to aim a rifle at his father in the restaurant below, escalating the imminent tragedy. Milo rises to kiss Sally as the shooting erupts, creating collective panic.

The film reveals that the Meanwhile City is a fantasy constructed by David after his wartime trauma and his sister's death. Emilia, cornered by gas, tries to flee but David triggers a deadly explosion by igniting the gas—killing himself and destroying the apartment. Emilia survives and, as paramedics arrive, she is spotted by Milo, bridging the two fractured worlds.
