New York City detective and sex-crimes specialist Adam Rourke has journeyed to Manila to help his friend, Miguel Ramos, investigate a chilling spate of murders: several young women found drained of blood, each marked by a identical 10 cm incision on the inside of the forearm. While Miguel suspects a homicidal maniac, rumors swirl of a strange blood cult at play. Adam goes undercover as a writer chasing the story of the latest victim, Maria Cortez, a hostess at the Barrio Club, owned by Calderone.

He steps into the club, brushing aside a pretty hostess, Theresa Theresa, and fixates on the club’s alluring dancer, Serena Serena. He and Calderone strike up a conversation about the article and the mystery surrounding Maria, while the rhythm of the club hides a darker truth.

That night, after a scuffle in his hotel, Adam defends himself and kills a man who attacked him. A seemingly homeless man, Herrera Herrera, is revealed to be a police liaison with a prosthetic leg, guiding Adam through the investigation. The tension tightens as Miguel’s sister, Sylvia Ramos Sylvia Ramos, drives Adam into the countryside, challenging him to take the case more seriously. On the return to the city, Theresa arrives home visibly inebriated, only to be stalked by a lurking humanoid monster, unseen by Adam. At the Barrio Club, Serena clutches her face and suddenly appears older before slipping away.

Miguel taps the authorities in Lima, learning that Calderone and Serena fled Peru two years earlier after Calderone’s name had been tied to the murders of several young women. Elena, another club hostess, is abducted by the same creature, leaving Adam increasingly convinced that Calderone is the killer. Serena, sly and evasive, tells Adam that Calderone “is a maniac and he’ll kill to protect his secret.” She introduces Adam to Louisa, a new hostess who is really Sylvia in disguise; Louisa has fallen for Adam and hopes to help him see through the deception. She notes that Serena’s wrist-to-elbow armbands bear Aztec or Inca symbolism, hinting at an ancient influence behind the murders.

The monster claims Elena in a hidden chamber beneath the Barrio Club. There, it moves Elena’s blood into a bowl set before Sylvia, who sits with a bright box behind her head. Serena sprinkles powder into the blood, and as the smoke clears, Serena appears youthful again. She asks Adam to come to her home and spins a web of lies, claiming Calderone killed his wife and coerces her to dance at the club. Adam, drugged by a drink, slips into unconsciousness. Meanwhile, Sylvia discovers the secret chamber and is seized by the creature.

Adam awakens bound to a tree. Serena explains that she was chosen “by her people” to become one of the “golden goddesses,” but the price is the blood of other women to sustain her eternal beauty. The blood, mixed with powdered plant roots and the “electrical energy of the sun” in a small container, sustains the ritual. Serena removes an armband to reveal a scar identical to those on the murder victims, but as she readies to stab Adam, she ages rapidly and flees to the chamber.

Freeing himself with the help of one of Calderone’s men, Adam and Miguel race to the chamber. They find the monster poised to kill Sylvia, but the confrontation erupts into a clash that sends the bowl of Elena’s blood sliding in front of Serena. Miguel fires at the creature, four times, with little effect, and the monster fights back, nearly strangling both men until Herrera strikes with his artificial leg, bringing the beast down. In the struggle, Adam and Miguel save Sylvia.

Serena dumps more powder into Elena’s blood. When the smoke clears, she has become an old woman once again, and, with a final handful of powder, only her clothing and armbands remain. The monster dies, revealed to be Calderone.

With the case closed, Adam shares a quiet farewell kiss with Sylvia before heading back to New York, bringing the memory of Manila with him as he returns to a familiar life.
