The film begins with a striking visual presentation of a gourmet meal being meticulously prepared. A voiceover establishes the existence of three distinct types of people—those above, those below, and those who fall—setting the tone for the grim dynamics within the pit.

Goreng wakes up in a stark concrete cell, bewildered by his surroundings. He quickly meets Trimagasi, an older inmate who reveals they are on level 48 of the pit, where their survival hinges on the leftovers from the prisoners above.

Trimagasi explains how the quality of food they receive depends on what the higher levels leave behind. As the food platform descends, Goreng watches in horror as they are forced to eat scraps touched by countless others, highlighting the brutality of their environment.

Goreng finds a perfect apple among the remnants and decides to save it for himself. However, he soon experiences a rising temperature in their cell, a sign of dire consequences if he refuses to return the food he hoarded.

In an attempt to bond, Goreng shares his story with Trimagasi, revealing he traded six months of his life for an accredited diploma. Trimagasi, serving a year, recounts how a bizarre accident with a knife sharpener led to a man's death that he brushes off as inconsequential.

As Goreng and Trimagasi struggle to navigate their dire conditions, Goreng attempts to convince higher-level prisoners to be more considerate with their rations. Trimagasi, however, warns him against such futile attempts, emphasizing the structure's inherent cruelty.

Goreng's feelings of dread intensify when a body unexpectedly falls into their cell. Shortly thereafter, Miharu, a bloodied woman with a heartbreaking mission, arrives seeking her lost son. Trimagasi reveals her grim strategy to ensure her survival in the pit.

Goreng and Trimagasi's situation worsens when they are transferred to level 171, where they face even greater challenges. As hunger sets in, Trimagasi begins to eye Goreng as a potential source of sustenance, causing panic and tension to rise.
