Phillip, Patrick Thompson, an Australian cop and Interpol cybercrime investigator, is left shaken after a brutal Hamburg case where a man consented to having his own penis cut off and eaten by his lover—the German Cannibal Adam Hunt. The disturbing details of that case echo through Phillip’s life, and his personal relationship frays under the weight of constant travel and difficulty with intimate connection; his girlfriend’s advances overwhelm him, and their volatile encounter ends with her leaving him, scrawling “pig” on his chest.

Back home, Phillip teams up with his partner, Nigel Matthew Le Nevez, to probe a fetish website that features morbidly obese women being held captive and fed fattening foods. The site’s encryption hints at a darker layer beneath the surface, and despite pushback from his superiors, Phillip travels to Toledo, Ohio to investigate the webmaster and hunt down a former site favorite known as Lucy Victoria Doyle. The journey deeper into the site’s twisted world reveals a cruel mastermind and a village of victims who may be more entangled than they appear.

In Ohio, Phillip identifies the sadistic webmaster as Michael Carter Alex O’Loughlin, who holds Deirdre Gabby Millgate captive in a ramshackle cottage in the woods. After consulting a local priest, Phillip traces Michael to the secluded residence, where the latter is preparing to feed Deirdre a thick slurry of eggs and weight gain powder. Phillip learns that Michael’s fixation with obese women has roots in a troubled childhood relationship with his overweight, immobile mother, who died when he was a child. This backstory helps explain not only the site’s perverse activities but also the personal motives behind Michael’s crimes.

As Phillip digs further, he uncovers the full extent of Michael’s fetishes: paying members can watch him feed and fornicate with obese women, and the site even lets them place bets on when each woman will die, using posted statistics on body proportions, blood pressure, and other medical indicators. The cottage becomes a grim stage where truth collides with fantasy, and Phillip eventually discovers Lucy’s decaying remains, confronting Michael with the grim revelation that he killed his mother and fed Lucy until she died.

A tense confrontation ends with Phillip shooting Deirdre, who still clings to love for Michael even as Phillip unveils the deception. The shots go off-screen, leaving the aftermath to the viewers’ imagination.

The final act slips into a stark and unsettling domestic tableau: Phillip is shown living in suburban bliss with Michael’s adoptive sister. He takes the sandwiches she packed for him and drives back to the cottage in the woods, where he eats with gusto and twists the knife of his power over Michael, who lies in a wheelchair and begs for mercy. He taunts Michael with a grim, lingering meal as the grim cycle of hunger and control continues.
