Dr. Henry Carter Kevin Spacey is a renowned Hollywood psychiatrist who treats some of the film industry’s brightest stars, each grappling with their own private crises. He lives in a large, luxurious house high above the city, a place that speaks of success even as it mirrors his personal fragmentation. Behind the aura of a best-selling self-help author, Carter remains disheveled and solitary, smoking marijuana at home, in his car, and behind his office door when he isn’t between patients. He repeatedly drinks himself to sleep, waking in his clothes but never crossing into his own bedroom, a ritual that hints at deeper wounds. Yet, despite his own struggles, he maintains a sharp, compassionate therapeutic stance, preserving incisive insight and strong doctor-patient bonds that keep his practice alive.

Among his patients and acquaintances, Patrick stands out as a high-powered talent agent who is both narcissistic and angst-ridden, and who harbors a germaphobic streak. Seamus, an actor with a dangerous dependence on various drugs and alcohol, is one of Patrick’s biggest clients; while Seamus isn’t technically Carter’s patient, the two share a drug dealer named Jesus, a detail that threads through the lives of several characters. Jack Holden, another celebrated figure, teeters on denial about a drinking problem, continuing therapy because he clings to a belief that his struggles stem from sex addiction. Kate, a poised and intelligent actress in her thirties, confronts shrinking career opportunities, partly because Patrick has painted her age as a professional obstacle. Her rock-star husband, self-centered and unfaithful, adds another layer of tension to Carter’s world.

Into this web enters Jemma, Carter’s newest patient—a troubled high-school student who is mandated to see a therapist after punching a mirror and cutting her hand. Referred to Carter by his father as a pro-bono case, Jemma shares a painful connection with Carter’s own late wife, whose death by suicide echoes in Jemma’s life. An avid moviegoer with clear ambitions, Jemma dreams of someday directing or making films.

Carter’s circle extends beyond the consulting room. He spends time with Jesus, a quirky pot dealer who appears almost as a confidant, and with Jeremy, a struggling young screenwriter who is loosely connected to Carter through the deceased wife’s family. Jeremy becomes romantically entangled with Patrick’s assistant Daisy and finds inspiration in Jemma, secretly stealing her private file from Carter’s office to fuel his own creative project. He writes a breakthrough screenplay about Jemma, and with Daisy’s help, catches Patrick’s eye, setting in motion a new momentum in the lives of all involved.

The pressure of fame and the fragility of trust collide under the glare of a televised spotlight when Carter experiences a breakdown on a live talk show.
