Kambilikandam Jose, Subair a drug baron in Kochi, travels to meet Pattakkalil Purushothaman “Appichai” Pilla, Vijayaraghavan one of South India’s largest drug dealers, hoping to secure a massive hike in his commission. Appichai arrives with his foster son Sethumadhavan “Sethu”, Saikumar. Appichai calls Sethu, who is on a cruise in Goa. Upon his return to Kochi, Sethu kills Jose with the help of IG Balagopal, Ramu. The murder triggers a strong wave of protests across the state, and the Chief Minister Bhargavan Menon, Janardhanan, orders a detailed investigation to uncover the truth behind the violence and the power network fueling it.

ACP Narendran “Nari”, Mammootty, is placed in charge of the inquiry. Widely known for his aggressive and belligerent approach, Nari begins by questioning Dr Ratnakumari, Urmila Unni, the assistant surgeon who performed the postmortem on Jose. Nari soon detects marks of a physical attack on Jose’s face that were not reported, and when he tries to reach Jose’s family, he discovers that all of them are dead the next day. Meanwhile, Sethu maintains an illegal business partnership with Jeevan, the CM’s son and one of the country’s most influential businessmen, complicating the investigation and creating dangerous pressure on Narendran’s path forward.

Hiding in an ashram named Karunyadeepam, under Rema Behan, Sethu runs a clandestine drug cartel. Narendran presses on and reaches Rema Behan, who is found dead the following day. The case becomes tangled as Narendran faces multiple obstacles, including Jeevan and several senior cops, and is eventually discharged from the investigation. Undeterred, Narendran uses a discreet video clip on Jeevan—provided by Sethu—to blackmail the Chief Minister into allowing him to resume the probe, insisting that the truth must come out.

Narendran then makes a bold move, arresting Sethu, who had threatened Appichai, after giving him 24 hours to reveal information. In a tense confrontation at a safe place, Helen’s brothers arrive and attempt to kill Sethu, but Narendran intervenes and kills them. Sethu fires at Narendran and wounds him in the arm, but Narendran replies by shooting Sethu in the leg and then triggering a car hanging from a garage ceiling to crash onto Sethu’s legs. Narendran escapes the chaotic scene with Shihabuddeen, Thomas Antony IPS, and Hamsa, leaving Sethu to bleed out as the pressure of the investigation reverberates through the corridors of power.
