International terrorist Charles Rane undergoes plastic surgery to change his identity as part of his plan to evade capture. The surgical team, however, is complicit in his escape, leading to a dramatic series of events.

As suspicions rise during the surgery, Rane becomes desperate and murders the surgeon to prevent his escape plan from being thwarted. This act sets off a chain reaction, leading to a police chase through the streets.

Former police officer John Cutter copes with the traumatic loss of his wife while training flight attendants and security personnel on handling terrorist threats. A tension-filled encounter arises between him and flight attendant Marti Slayton during one of his sessions.

Cutter meets his old friend Sly Delvecchio, who presents Cutter with an offer to become vice-president of the anti-terrorism unit at Atlantic International Airlines. After some hesitation, Cutter is persuaded during a lunch with Delvecchio and the airline's president.

Cutter embarks on his first flight with Atlantic International, heading to Los Angeles. To his surprise, he finds Marti Slayton working as a flight attendant on the same flight, rekindling their earlier tension.

Charles Rane, under FBI custody, is aboard the same flight along with his disguised accomplices. When a headcount conducted by Marti identifies Cutter, the tension in the air rises significantly.

Disaster strikes when Sabrina Ritchie, posing as a flight attendant, executes the FBI agents guarding Rane, allowing chaos to ensue. As Rane's crew takes control of the flight, Cutter, caught unawares, realizes the gravity of the situation.

Cutter, who was in the lavatory during the takeover, attempts to fight back against Rane's men. In the chaos, Rane kills a passenger, which leaves Cutter feeling immense guilt for not being able to protect them.

In a tense confrontation, Cutter and Marti manage to flee to the plane's lower deck, where they initiate a fuel dump. This action forces the plane to divert to a small airfield in Louisiana, providing a glimmer of hope amidst the chaos.

Rane reaches out to local sheriff Chief Biggs, attempting to negotiate the release of hostages in exchange for fuel and clearance to take off. Meanwhile, Rane cunningly labels Cutter as a traitor, creating tension in the situation.

Despite being labeled a traitor, Cutter overcomes the sheriff's deputies and commandeers a police motorcycle to rescue the remaining hostages. His determination showcases his heroism and refusal to abandon those in danger.

Cutter engages in a fierce battle with Rane's henchmen at a nearby fairground, which escalates into a showdown with Rane himself. With the police closing in, Rane threatens to execute hostages still on the plane, raising the stakes immensely.

In a tense standoff, Cutter, Henderson, and Chief Biggs devise a plan involving FBI snipers to apprehend Rane. However, the plan goes awry when a sniper's shot accidentally hits Rane's escort, allowing Rane to regain control of the plane.

In a daring act, Cutter jumps onto the moving plane's landing gear to confront Rane. Inside, he fights Rane's henchmen and ultimately engages Rane in a brutal fistfight that leads to explosive cabin decompression.

With Rane defeated and plummeting to his demise, the plane manages to land safely in Louisiana again. In the aftermath, Cutter and Marti share a peaceful moment, hand in hand, leaving behind the turmoil of their harrowing experience.
