Max Matheson survives a severe car crash that kills his fiancée Kayla. The crash haunts him with guilt and trauma, while the other driver is held responsible. He begins psychological treatment with Dr. Beaumont to cope with the aftermath.

Max's father, Jack, reopens the Mayflower Department Store in New Orleans and hires Max as a security guard. He is introduced to the store's manager Keller Landreaux, buyer Jenna McCarty, and vice-president Ryan Parker. Max agrees to take the job and starts settling into the new role.

Before his first shift, Max sees a vision of a dead woman in a mirror and witnesses Jenna’s naked reflection ripping off her head. The eerie ability foreshadows deaths through the mirrors. Jenna later dies from a shard of glass after being frightened by her reflection.

On the next night, Max sees Ryan’s reflection being eviscerated and tries to warn him. He is unable to prevent Ryan’s death, which occurs in the same manner. The visions confirm that Max can foresee deaths through the mirror world.

Max calls Jack to say he is quitting, then rushes to his father’s house only to find Jack’s face covered in cuts. Jack’s reflection nearly harms him, but Max intervenes and saves him. This close call convinces Max to stay at the Mayflower and protect his father.

Back at the Mayflower, Detectives Huston and Piccirilli question Max about Jenna’s and Ryan’s deaths. He denies involvement, and the detectives remain suspicious while allowing him to continue working. The weight of the mystery grows as they hint at a supernatural thread tied to the mirrors.

Max goes to the main mirror to learn who the ghost is and follows a guiding flashlight. He finds a box with Eleanor Reigns’s employee ID and a missing-person flyer, then contacts Eleanor’s sister to gather more details about her disappearance.

Max and Elizabeth uncover that surveillance files from the night Eleanor disappeared were deleted. The blackout points to a cover-up by someone at the Mayflower. The discovery deepens the mystery and links the case to the store’s darker past.

They learn from Henry Schow, the last to access the files, that Eleanor was murdered after being drugged at the groundbreaking party. Jenna drugged her drink, Keller raped and killed her, and Henry found Keller burying the body in the basement crawlspace and forced him to delete the footage.

After Eleanor's death, her spirit, trapped in the mirror world, begins seeking revenge on those responsible. She manipulates Henry's reflection to eat broken glass and to kill Jenna and Ryan. Max realizes his near-death experience has given him the ability to see spirits.

Max and Elizabeth search the Mayflower basement for Eleanor’s body as Keller chases them. He nearly strangles Elizabeth, but Max saves her and recreates the near-death moment. Eleanor then pulls Keller into the main mirror and kills him, leaving Max and Elizabeth shocked.

At the police station, Henry confesses his involvement in the cover-up. The interrogation room’s one-way mirror cracks as Henry confronts Eleanor’s presence in the reflection. Eleanor kills Henry as the screen cuts to black, implying the haunting continues.
