Dheeraj's heart is missing because he made a dark pact with the evil spirit Rahasur. In exchange for Rahasur helping him ruin Radhika's life, Dheeraj agreed to give his heart to the spirit after his death. This supernatural deal explains why the autopsy found his cardiovascular system intact but his heart gone.
