As the story unfolds, Max Payne (Mark Wahlberg) finds himself desperately fighting for survival in a frozen river, struggling to break free from its icy grip. In a haunting voice-over, he reflects on the events that led him to this moment, taking the viewer back one week earlier when he was a dedicated clerk in the Police Department’s cold-case unit. Max is consumed by an obsession to identify the vicious killer who took the lives of his wife, Michelle, and their newborn daughter.

Seeking new leads, Max crashes a party hosted by his informant, Trevor Duncan, and encounters Natasha Sax (Mila Kunis) and her sister, Mona. In a twisted turn of events, Natasha makes a provocative advance towards Max, even suggesting she could take on the role of his deceased wife. Visibly disturbed, Max asks her to leave, but tragedy strikes when Natasha is brutally attacked and killed by an unseen, winged entity in an alley.

The following day, Max, alongside his former partner Alex Balder, begins to piece together the mystery surrounding Natasha’s murder. Max’s path leads him to confront B.B. Hensley (Beau Bridges), his mentor, as he grapples with loss and betrayal. Despite the mounting pressures, he manages to build a tenuous partnership with Mona, who initially suspects him of wrongdoing. Together, they pursue Owen Greene, the last person to communicate with Natasha before her death, only to find him met with a similarly tragic fate.

From the underbelly of crime, they discover that Jack Lupino, a vicious crime boss, is spiraling out of control due to his dependence on a dangerous drug called Valkyr. As Max digs deeper, he learns that this substance was developed by Aesir Pharmaceuticals for military use and is linked to his wife’s death.

Confronting Jason Colvin, a former supervisor of Michelle’s, Max uncovers shocking revelations about Valkyr’s horrific side effects, which led to Michelle’s inquiries into the project. In a desperate bid to silence everyone involved, Colvin is murdered right before Max’s eyes, leading to an adrenaline-fueled escape from the corporate chaos that surrounds him.

Later, as Max watches a shocking video reveal the gruesome reality of Aesir’s drug experiments, he feels the weight of the world crashing around him. He learns that Jack Lupino’s maddened spree may stem from the very drug designed to enhance soldiers’ combat prowess. Despite the odds, Max hunts Lupino down, leading to a climactic showdown in Ragna Rock, where he battles Lupino’s henchmen and ultimately confronts the demonized version of his former mentor.

After being betrayed by B.B., Max finds himself near death, only to return stronger, fueled by vengeance and the hallucinations of Valkyries haunting him. In a high-stakes finale at Aesir Pharmaceuticals, Max engages in a brutal shootout but is saved by Mona just in time to finish what he started. A final confrontation leads to B.B.’s demise, offering Max a fleeting moment of clarity as the skies above him clear, revealing a glimpse of his wife and daughter.

In a poignant post-credits scene, Max crosses paths with Mona at a bar, sharing a silent understanding over a newspaper article documenting Aesir’s profits. This exchange hints at the lingering shadows of their past and the demons that continue to haunt them.
