Cordell, impaled by a pipe and plunged into water, returns as the undead Maniac Cop. He commandeers a junked police cruiser and begins a new killing spree through New York City. His unstoppable rampage sets the city on edge as he hunts civilians and officers alike.

Cordell infiltrates a convenience store during a robbery and coldly shoots the clerk. The thief is killed in a subsequent shootout with responding officers. The massacre marks the start of his renewed terror.

Deputy Commissioner Doyle orders Officers Jack Forrest and Theresa Mallory back on duty and has them undergo psychiatric evaluation. The move signals the police’s determination to treat Cordell as a real threat again. Tensions rise as they rejoin the force.

Cordell tracks and fatally stabs Jack Forrest at a newsstand, delivering a brutal blow to the investigation. Theresa Mallory becomes convinced that Cordell is still alive and plotting revenge. The killing reinforces the sense that Cordell's menace endures.

To protect Commissioner Pike's memory, the police refuse to inform the public about Cordell's recent actions. Theresa begins planning to warn the public by appearing on a talk show. The department's silence deepens the fear surrounding his return.

Cordell gains entry into Sing Sing using Blum's paperwork and kills a guard for keys. Doyle's PA announcement confirms the reopened case, accelerating Cordell's advance. Cordell, locked in battle with the inmates who mutilated him and Turkell, crashes through a wall into a waiting bus, which explodes.

Cordell is buried with full honors beside 19 officers he murdered as Susan and McKinney attend the funeral. McKinney throws Cordell's badge into the grave and speaks of justice, while the camera reveals Cordell's hand bursting from his coffin in a final jump scare.
