Matthew Cordell, once a respected lawman, returns from death with a vengeance, re-igniting his brutal crime spree across New York City. After a brutal impalement and a plunge into the water, the now-undead figure commandeers a vandalized police cruiser and resumes his murderous path. His first grisly act after his return takes place at a grocery-style convenience store, where he slays the clerk in cold blood; the accompanying thief is promptly blown away by a gunfight with the police. As Cordell stalks the streets, the department responds by putting back on duty the longtime officers tasked with stopping him, including Jack Forrest and Theresa Mallory. Deputy Commissioner Edward Doyle orders them into a psychiatric evaluation overseen by Susan Riley. While Jack clings to a belief that Cordell is dead, Theresa remains convinced that the killer is very much alive and plotting a return.

A fateful clash unfolds at a newsstand when Cordell fatally stabs Jack, cementing the sense that the threat is far from over. In a murky act of corrupt cover-up, Doyle and other officials refuse to tell the public the true nature of the danger, hoping to keep Cordell’s threat quiet. Theresa, undeterred, takes to a talk show to warn the audience, while separate murders ripple through the city—most notably a traffic cop slain while attempting to tow a car. The crime investigation tightens its grip as the man responsible for the drive-away murders is arrested, raising the tension that Cordell’s reign of terror may be far from finished.

On a tense ride to a hotel, Theresa is pursued by Cordell, who forces a taxi driver to stop and removes Susan Riley from the scene. Susan is left trapped in a car as Cordell attempts to break her, but a wrenching turn of events leads to Susan being found by the authorities after a violent confrontation. Meanwhile, a stripper named Cheryl, who has already endured danger, becomes entangled when Steven Turkell, a man with a grim history of attacking exotic dancers, crosses paths with Cordell. Cordell arrives, murders two officers called by Cheryl, and helps Turkell slip away. Turkell’s presence compounds the danger, and he is identified at a strip club by Cheryl to Susan and Sean McKinney, leading to Turkell’s arrest and confinement.

Cordell then erupts into the heart of the city’s police precinct, killing nineteen officers and freeing Turkell and several other inmates. With Susan as a hostage, Turkell and Cordell hijack a prison bus and push toward Sing Sing, where Turkell believes Cordell intends to break an entire army of criminals loose. Sean McKinney and Doyle pursue, and McKinney persuades Doyle to reopen Cordell’s case and rebury his casket with full honors in a bid to appease the killer—an attempt that proves tragically ineffective.

Invading Sing Sing, Cordell gains entry using Blum’s paperwork and eliminates a guard to obtain keys. In the death row area, Doyle’s broadcast over the prison PA reveals that the case has indeed been reopened. Cordell, sensing the shifting tides, abandons Turkell, Blum, and Susan and winters deeper into the prison’s maze. There, enemies who mutilated him in life unleash a Molotov-fueled ambush, and Cordell fights back with ferocity, killing the assailants and triggering a chaotic escape that ends with a fiery crash from the bus below.

Time passes, and Cordell’s funeral is held with full honors, joined by Susan and McKinney as they reflect on the pursuit of justice. As the ceremony reaches its close, McKinney drops Cordell’s badge into the grave and delivers a stark meditation on the thin line between a cop and a “Maniac Cop.” The camera lingers on the grave as a final, chilling twist unfolds: Cordell’s hand bursts from the coffin, snatching his badge in a last, unsettling jump scare before the credits roll.
