Three years pass, and on the day he is slated to visit his father and Janet Gill at home, Gilbert makes a brutal break for freedom. He escapes the sanitarium, murders several employees and patients along the way, and hijacks a pickup truck as he leaves the grounds. He returns to the family house, fashioning makeshift weapons from ordinary objects, then heads to a horror shop where he steals a clown mask and slits the clerk’s throat, a chilling display of the unmoored violence now fueling his flight.

Meanwhile, his father and Officer Spiegel begin to close in, but the tension shifts when family ties pull Gilbert toward his sister’s world. At the Spiegel house, Janet Gill, D.J. Spiegel, and their friend Brooke host a late-night sleepover and horror-film binge, joined by Brooke’s boyfriend and Janet’s friend group, including Lance.
