Keeper ends with the true purpose of the isolated cabin finally revealed. Throughout the weekend Liz has been unsettled by strange noises, a blood‑stained box and a homemade cake that tastes “like crap.” The cake is not a dessert at all—it is a sedative meant to lull the chosen victim so the forest‑dwelling entities can devour them. When Liz, driven by an unexplained compulsion, eats the whole cake, she unknowingly offers herself to the creatures that have been haunting the woods.

Later, after a brief escape into a river that leaves her unconscious, Liz awakens in the dark basement surrounded by the beings. They present her with a honey‑preserved, bullet‑riddled head—the corpse of a woman they worship as their “mother.” Malcolm, who is revealed to be more than two centuries old, explains that the creatures only accept women who resemble that original mother; Liz is a perfect double. The creatures therefore see her not as prey but as a new matriarch, the one who will sustain them.

Malcolm’s plan was to use Liz as the next caretaker, feeding the entities fresh souls while he reaps the benefits of their immortality. His cousin Darren is already dead, and Malcolm meets his own end when Liz, now understanding the horror, drowns him and submerges his head in a pot of honey—a ritual offering to the fae‑like beings. The final image shows Liz carving a heart into the cabin’s window, mirrored by an invisible hand on the opposite side, suggesting that the forest has accepted her as its new “keeper.”

Thus the film closes with Liz transformed from a passive girlfriend into the reluctant guardian of a centuries‑old, blood‑thirsty cycle. She inherits the same power and burden Malcolm once held, implying that the ritual will continue, now with her at its center.
