In North Africa, Frank Cotton acquires a mysterious antique puzzle box from a dealer. The dealer's chilling question, 'What's your pleasure, Sir?' foreshadows the horrors that will follow as Frank takes the box back to England.

Back in his English home, Frank becomes obsessed with solving the puzzle. In a horrifying twist, he triggers the box's dark magic, resulting in chains that rip into his flesh and summon the Cenobites.

After their summoning, the Cenobites inspect Frank's dismembered body. They grotesquely reassemble his face and return the puzzle box to its original form, leaving no trace of the horror that occurred.

Larry Cotton moves back into the old house he shared with Frank, accompanied by his wife, Julia. Unbeknownst to Larry, Julia is keeping a dangerous secret about her past affair with Frank.

While moving a mattress, Larry accidentally injures his hand on a protruding nail. His blood drips onto the attic floor, unknowingly nourishing the hidden remains of Frank, who begins to awaken in agony.

As Frank awakens, he transforms into a grotesque, partially restored creature. Julia finds him in the attic, and despite her initial shock, she chooses to aid him, feeling compelled by their past.

Julia begins to lure unsuspecting men to the attic, allowing Frank to feed on them to regain his strength. Their twisted relationship deepens as Frank reveals his plans to escape with Julia.

Kirsty Cotton, Larry's daughter, becomes suspicious of Julia's actions when she observes her bringing home a strange man. Her curiosity leads her to spy on the attic, where she witnesses a horrifying scene.

Just as Frank tries to attack Kirsty, she manages to grab the puzzle box and throws it out the window while escaping. This act signifies her growing awareness of the box's significance and the lurking dangers.

After believing her experiences to be a nightmare, Kirsty finds herself in a hospital where she accidentally solves the puzzle box. This mistake opens a portal to a sinister dimension, bringing the Cenobites after her.

Kirsty makes a frantic deal with Pinhead, the leader of the Cenobites, promising to lead them to Frank in exchange for her freedom. She realizes that her life is intertwined with the horror that has engulfed her family.

Upon returning home, Kirsty uncovers a gruesome truth: Frank has killed her father and is masquerading in his skin. This shocking revelation leads her to confront the horrifying reality of her family's demise.

A deadly standoff ensues between Kirsty and Frank, culminating in Julia’s fatal injury during Frank’s rage. As tension peaks, Frank chillingly reveals that Larry was 'dead inside anyway.'

The Cenobites return to claim Frank as he attempts to attack Kirsty. In a dramatic twist, chains erupt from the box, binding him as he utters his final words, 'Jesus wept,' before being torn apart.

As Kirsty tries to escape with her boyfriend, they face another attack by the puzzle box's creature. In a final act of desperation, she attempts to destroy the box, only to see it find its way back into the hands of a mysterious vagrant.
