What Happens in Hellraiser: Full Plot Summary

The whole story of Hellraiser (2022) explained, from the opening scene to the final frame: every key event, every twist and what it all adds up to. Read it as a quick recap before the sequel, to check a scene you missed, or to finally understand what really happened.

During a lavish gathering at the mansion of the opulent millionaire Roland Voight, a sex worker named Joey stumbles upon a mechanical puzzle box that Voight insists she solve. After successfully figuring it out, she is quickly met with a deadly surprise—a blade springs from the box and fatally wounds her, subsequently unleashing a portal from which chains emerge and brutally dismember her.

Fast forward six years, we meet Riley, a recovering addict who lives with her brother Matt, his boyfriend Colin, and their roommate Nora. Encouraged by her boyfriend Trevor, Riley agrees to break into an abandoned storage facility, where they discover the mysterious puzzle box. After a confrontation with Matt leads to her storming out, Riley finds herself in an empty park where she solves the box but narrowly escapes injury. This triggers the appearance of the Cenobites, a sinister group of mutilated beings who demand she make another choice for sacrifice.

Matt finds Riley unconscious and tries to rouse her, only to inadvertently slice himself on the box's blade, prompting his mysterious disappearance. Grieving and blaming the box for Matt's fate, Riley, along with Trevor, seeks out Serena Menaker, Voight’s former lawyer, who reveals the box’s dark history. A reckless attempt by Menaker to seize the box results in her getting cut and falling victim to the Cenobites.

Determined to uncover the truth, Riley ventures into Voight's deserted mansion where she discovers his journals. They reveal that each configuration of the box has dire consequences—requiring a victim to be marked for the Cenobites to take, and each completion promises a "gift" from Leviathan, the arbiter of Hell. Riley is horrified to see an apparition of Matt, discovering he has been flayed in the process.

As Riley explains her findings to Colin, the still-living but mutilated Voight unexpectedly attacks, leading to Nora’s death by the Cenobites. In their escape attempt, the group crashes their van and finds themselves back at the mansion. Confronted by the Priest, the leader of the Cenobites, Riley faces a horrific ultimatum. She is enticed with a chance to resurrect Matt if she submits to sacrificing two more individuals. After a tense struggle, Trevor is injured by the Chatterer, one of the Cenobites, and Riley is tragically cut by the blade herself.

Riley works to solve the subsequent configuration and manages to rend the Chatterer to pieces, but as events unfold, the true treachery reveals itself. It’s discovered that Trevor has been in collusion with Voight to provide victims for the box. In a desperate attempt to ensnare the last Cenobite, Asphyx, Riley and Colin are thwarted when Voight re-emerges, stabbing Colin with the box himself.

Voight then shares his harrowing story of seeking extreme pleasure after completing his sacrificial tasks, only to be relegated to a life of pain from the contraption he bears. He completes the final configuration, seeking liberation from his torment, but summoning Leviathan heftily confirms his tragic fate.

As sacrifices come to a conclusion, the Cenobites present Riley with an offer to resurrect Matt. Recognizing the inherent twisted nature of such "gifts," she chooses instead to embrace her grief, denouncing the bargain. By opting to live with her burden, she is given the gift of "lament." As the Cenobites disappear and the box returns to its cube shape, Riley and Colin walk away from the mansion, leaving her to ponder if she truly made the right decision. The gruesome fate of Voight awaits him, as he transforms into a new Cenobite within Leviathan’s grasp.

Hellraiser Plot Explained: Your Questions Answered

The questions people ask most about Hellraiser (2022): what happens in the confusing scenes, why the characters do what they do, the twists, hidden details and what the ending means. Each answer contains spoilers.

Q1

Why does Joey die immediately after solving the puzzle box?

Joey dies because solving the box's configuration summons the Cenobites and requires a victim. When she successfully solves it, a blade springs from the box, killing her and unleashing the chains that dismember her body. The box demands sacrifice as payment for opening the portal.

Foreshadowing: The opening scene establishes that solving the box is not a reward but a trigger for death.

Q2

What is Leviathan in Hellraiser?

Leviathan is the ancient entity and arbiter of Hell that rules over the Cenobites. It is the power behind the puzzle box, granting the 'gifts' that come with each configuration. Those who complete the box's final configuration ultimately answer to Leviathan, as shown when Voight summons it and is transformed into a Cenobite.

Q3

Why does Matt disappear after cutting himself on the box?

Matt disappears because the box's blade marked him as a sacrifice. When he accidentally cuts himself on the blade while trying to wake Riley, he becomes marked for the Cenobites. Riley later discovers him as a flayed apparition, indicating he has been taken into Hell and tortured.

Foreshadowing: The box's blade has already killed Joey and injured Riley, establishing it as the mechanism for marking victims.

Q4

Why was Trevor working with Voight?

Trevor had been colluding with Voight to supply victims for the puzzle box. Voight, unable to find victims himself in his weakened state, used Trevor as an accomplice, likely promising him some form of reward or escape from consequences. This betrayal explains why Trevor encouraged Riley to break into the storage facility where they found the box.

Foreshadowing: Trevor's insistence on the break-in seems motivated by more than just thrill-seeking, hinting at prior knowledge.

  • trevor
  • betrayal
  • voight
  • collusion

Q5

Why does Riley turn down the chance to bring Matt back?

Riley refuses the Cenobites' offer because she recognizes their gifts are inherently twisted. Resurrecting Matt would require sacrificing two more innocent people, and accepting such a bargain would corrupt her just as the box has corrupted everyone else who used it. She chooses to live with her grief rather than become complicit in more murder.

Foreshadowing: The film establishes that every 'gift' from the box comes at tremendous moral cost, as seen with Voight's suffering.

Q6

What is the gift of lament that Riley receives?

The gift of lament is the ability to feel and carry grief without being consumed or corrupted by it. By rejecting the Cenobites' twisted bargain, Riley is granted this burden rather than a hollow resurrection or eternal torment. It represents choosing to live authentically with loss rather than escaping through supernatural deals.

Q7

What happens to Voight at the end of Hellraiser?

After completing the final configuration of the puzzle box to escape his constant pain, Voight summons Leviathan and is transformed into a new Cenobite. His decades of using the box and sacrificing victims finally result in his own transformation, becoming one of the very beings he once commanded.

Foreshadowing: Voight's journals foreshadow that using the box repeatedly leads to becoming bound to Leviathan's service.

Q8

How does the puzzle box work in Hellraiser?

The puzzle box, called the Lament Configuration, has multiple configurations that each trigger different outcomes. Each completion requires a victim to be marked for the Cenobites to take. Completing all configurations summons Leviathan and grants what appears to be a wish, but the 'gifts' come with horrific consequences, the user becomes bound to Hell.

Foreshadowing: Voight's journals reveal the box's rules and the escalating consequences of each configuration.

Q9

Why does Voight stab Colin with the box?

Voight stabs Colin to use him as a sacrifice to complete the final configuration and summon Leviathan. Having already sacrificed countless victims over decades, Voight needed one more to attempt his escape from the box's torment. Colin becomes the final victim as Voight completes the configuration.

Foreshadowing: Voight's journals reveal he has been making sacrifices for years, establishing his willingness to kill again.

Q10

Why do the Cenobites kill Nora?

Nora is killed by the Cenobites because she was present during the confrontation at Voight's mansion and became collateral damage in the Cenobites' pursuit of sacrifices. When the mutilated Voight attacks Riley and Colin, Nora is caught in the crossfire and taken by the Cenobites, demonstrating that the box's violence extends to anyone nearby.

Foreshadowing: The Cenobites have repeatedly killed anyone who comes near the box or becomes involved with it.

Ending explained

What Happens at the End of Hellraiser?

How Hellraiser (2022) ends, in short: the final scene, the last twist and what it means for the characters. The full ending explained, with every spoiler, lives on its own page.

Read the full ending explained

The ending of "Hellraiser" presents a grim but thought-provoking conclusion. Riley, having managed to resist the Cenobites' temptations and refusing their offer of a boon, chooses to accept her fate and the pain that comes with her past and her losses. Her decision to pick the Lament configuration signifies her acceptance of the suffering associated with her grief and addiction, and she deliberately rejects the chance of resurrection or any other worldly reward. As a result, the Cenobites disappear, and Riley walks away with Colin, having dropped the puzzle in a symbolic act of letting go of her pain, her guilt, and her past. The ritualistic torture and the Cenobites themselves serve as metaphors for the destructive nature of addiction and the personal sacrifices it demands. For Riley, facing this darkness ultimately means choosing a life without illusions of escaping her suffering or returning what she has lost. Meanwhile, Voight, who also seeks more than what life can give, ultimately succumbs to the Leviathan, being transformed into a Cenobite himself. The film leaves us with a haunting reminder that the pursuit of excess and desire often leads to eternal torment, and true relief only comes from acceptance and relinquishing the illusions of control. Riley's rejection of the Cenobite's offer underscores her understanding that some wounds may never fully heal—and that moving forward requires accepting that pain as part of her life’s journey. The conclusion solidifies the film's message: that confronting and accepting one's suffering is the path to peace, even if it means forever carry the scars of what was lost."
