Travis Clyde Tuckton Elliot V. Kotek is released from prison in 2003 after serving time for involuntary manslaughter during a carjacking, and he ends up shacking up with his disabled grandfather in the old shoemaker’s secluded West Virginia home. Grandpap Jake Martin Dick Mullaney decides to teach Travis everything he knows, starting with the family tradition of headers—the act of having sex with a hole drilled into a skull. Travis soon picks up a hitchhiker, and under Jake’s wary supervision, loses his head humping virginity to her. A violent turn arrives when Travis kills a relative of a neighbor who had argued with Jake, and he vows to seek revenge on all who have wronged their family, declaring, and almost echoing a grim creed, “An eye for an eye, and a head for a head.”
A parallel thread follows Stewa r t Cummings Jake Suffian, an ATF agent who has turned to drug trafficking to pay for his girlfriend Kathy’s medicine. The two storylines gradually collide as Stewart investigates the rising chain of header victims, with clues leading him toward Travis. After killing and robbing the two drug dealers he was working for, Stewart picks up another hitchhiker and asks her about Travis. The hitchhiker tells him that Travis may be living with his grandfather, and provides directions to Jake’s cottage.
At the cottage, the tension erupts when Travis kills Thibald Caudill—who Jake claimed stole valuable land from their family and who was also linked to the deaths of Travis’s parents (covered up as a car accident). Stewart, arriving unexpectedly, walks in on Travis performing a header on Thibald and shoots both Travis and Jake, ending their risky alliance. He hurries back to his office, only to face arrest for murdering the drug dealers, one of whom was an undercover officer. A brutal struggle follows, culminating in Stewart shooting his superior and the arresting officer.
Returning home, Stewart discovers Kathy doing cocaine and having sex with her doctor, exposing that she had been faking her illness to secure drug money all along. Stewart’s grip finally slips as he kills the doctor, shoots Kathy in the knees, and retrieves a drill, signaling a grim, escalating plan for giving Kathy a header. The film weaves together the fates of two men bound by blood and crime, tracing a downward spiral of violence, loyalty, and a vendetta that consumes everyone around them.
