Stacey, [Tammin Sursok], Melody, [Alicia Lagano], Brian, and Sanjay, [Sunkrish Bala], are a group of college students traveling through the Ozark Mountains to study rural American customs and legends for a history project. Their journey introduces them to a string of eccentric, often deformed locals as they move from town to town. When their SUV breaks down with a flat tire, they seek help from Jeremiah, [Duane Whitaker], an elderly blind man whose speech trails into incoherence, and they accept a spare from him—an exchange shadowed by the fact that Brian, the group’s more abrasive member, underpays him.
The students stumble into a revival meeting, hearing whispered rumors about the Albino Farm. They press on to the small town of Shiloh and begin searching for the fabled farm, ignoring warnings from locals who carry their own marks of hardship. They encounter Levi, [Chris Irvine], and his two deaf friends, who offer to guide them to the Albino Farm for twenty-five dollars and, for Melody, a provocative incentive.
At the gates, a violent quarrel erupts between Levi and Brian, and the two sides collide. Levi and his companions drive off, leaving the students to fend for themselves. Inside an old house near the property, Brian spots a figure and pursues it, with Melody following. Brian is captured by a group of deformed and violent inhabitants living on the farm and is killed. As the others search for him, Melody becomes separated from the group and is killed in turn.
Meanwhile, Sanjay and Stacey continue their search for their missing friends and eventually reach the farm. They uncover evidence of earlier victims and are soon pursued by the same disturbing inhabitants. They are captured and wake up in an underground cave with their arms sewn together. They manage to tear themselves apart and attempt to escape, fighting off their captors along the way.
During the escape, Sanjay sacrifices himself by triggering an explosion that kills him and several attackers, leaving Stacey unconscious. When she comes to, she finds a revival meeting tent and seeks help. Inside, she discovers that the preacher is an albino, and the rest of the congregation displays a range of deformities. Stacey, overwhelmed by the scene, begins to laugh uncontrollably as the congregation breaks into song.
