Carlito, the head of a powerful Mexican-American crime syndicate, decides to eliminate Don Kim, the leader of a Chinese mafia faction threatening his operation. He sees the hit as necessary for the security of his organization, little realizing the severe consequences that will follow.

Carlito entrusts the dangerous task of killing Don Kim to his skilled hitman, Chev Chelios. This decision sets off a chain of violent events and stirs a fierce retaliation from the Chinese mafia after the hit is successfully carried out.

Ricky Verona, a small-time crook and longtime foe of Chelios, sees an opportunity to exploit the situation. He conspires with Carlito, betraying Chev's trust in an attempt to settle old scores and bring chaos to the crime syndicate.

Verona infiltrates Chelios' apartment and injects him with the lethal 'Beijing Cocktail.' This synthetic drug begins to shut down Chev's body by depriving it of adrenaline, leaving him with only a short time to act.

Chev engages in an outrageous public display of affection with Eve on the bustling streets of Chinatown, desperately trying to keep his adrenaline levels up. This daring moment draws the attention of many onlookers as he fights for survival.

In a last-minute change of heart, Chev decides to spare Don Kim's life, urging him to leave Los Angeles. This act reflects Chev's moral conflict amid the chaos unleashed by the earlier decisions.

Chev arranges a rooftop meeting with Carlito, Verona, and their gang, hoping to confront them about the false antidote they offer. Tensions rise as Don Kim's Triads join the fray, leading to a chaotic shootout.
