The story follows Carlito (Carlos Sanz), the head of a powerful Mexican-American crime syndicate based in Los Angeles. As tensions rise due to the threat posed by a heavily armed faction of the Chinese mafia, Carlito makes a grave decision to eliminate their leader, Don Kim (Keone Young), through a contract killing. He entrusts this dangerous task to his most skilled hitman, Chev Chelios (Jason Statham). However, Carlito’s judgment proves to be a mistake, triggering a violent wave of retribution from the Chinese, far exceeding his expectations.

Feeling the weight of his decision, Carlito considers the hit “ill-advised” and seeks to placate the Chinese by disavowing responsibility. In a bid to relieve the mounting pressure, he promises to remove rogue elements from his organization that acted without his sanction. Meanwhile, Ricky Verona (Jose Pablo Cantillo), a small-time crook and long-standing adversary of Chelios, seizes the opportunity for revenge. Unbeknownst to Chelios, Verona conspires with Carlito, exploiting Chelios’ misplaced trust in his boss.

The plot thickens as Verona breaks into Chelios’ apartment and injects him with the lethal “Beijing Cocktail,” a synthetic drug that starves his body of adrenaline, putting Chelios’ life at grave risk. Awakening to a grim reality, Chelios discovers a recorded message revealing he has roughly an hour left before the poison ceases his heart function. In desperation, he contacts Doc Miles (Dwight Yoakam), a physician with ties to the Mafia, who advises him that to survive, he must keep his adrenaline levels elevated through continuous excitement or procure artificial adrenaline.

With his life hanging in the balance, Chelios embarks on a frantic mission to avenge himself against Verona while racing against time. The entirety of the film unfolds over the course of a single day, showcasing Chelios attempting to track down Verona and his street gang with the help of his flamboyant street contact, Kaylo (Efren Ramirez). He also rushes to rescue his girlfriend, Eve (Amy Smart), from the clutches of Verona’s henchmen. In an outrageous bid to keep his adrenaline surging, Chelios engages in a public display of affection with Eve right in the bustling streets of Chinatown, under the watchful eyes of hundreds.

As the climax builds, Chelios decides to spare Don Kim’s life, urging him to flee Los Angeles. Eventually, he orchestrates a rooftop meeting with Carlito, Verona, and their cronies, who offer him a false antidote. The confrontation escalates when Don Kim and his Triads arrive, igniting a chaotic shootout that decimates many of Carlito’s men. In a shocking twist, Carlito meets his end at the hands of the cunning Verona, who attempts to escape aboard Carlito’s helicopter.

The film culminates in a breathtaking fight as Chelios confronts Verona mid-air, leading to a dramatic fall from the helicopter thousands of feet above the city. In a final showdown, Chelios breaks Verona’s neck during their descent. He then makes a heartfelt call to Eve, apologizing for not returning to her.
