A decade after the events of Adulthood, Sam Peel has traded the old life for something quieter, building a fragile normal with his girlfriend Kayla and their two children while continuing to support Alisa and her teenage daughter, whose father Sam killed years earlier. Alisa forgives him, but she asks for something more: she wants Sam to tell her daughter what really happened to Trife. The past looms when Sam overhears a girl describe an assault by a man named “Mooks,” a detail that will pull him back into a dangerous world he thought he’d left behind.

Soon, the danger intensifies. Sam’s brother Royston is shot during a performance, and an envelope addressed to Sam is left at the scene by the gunman Drew. In the hospital, Royston’s bandmate Henry reveals that Drew was sent to target Royston, and he hands Sam the envelope’s contents—a single address that Sam must visit alone. The trip to that address becomes a test of nerve as Sam encounters Janette, a woman who spills coffee and then seduces him, filming their encounter. The address itself is a sprawling mansion where Sam is quickly outnumbered, beaten, and brought to a basement where the criminal Daley confronts him on behalf of Mooks, the shadowy boss.

Sam refuses to join Daley’s gang, and the confrontation turns into a pulse-pounding escape that’s only the beginning of a larger feud. Got by, Sam is captured again but rescued by Henry just as Hugs, a ruthless thug, issues a warning with a vicious strike. Sam reaches out to Kayla to protect his family, but a shocking turn follows: Kayla discovers a DVD of Sam with Janette, and she leaves in anger, severing their trust.

Desperate, Sam seeks help from Buds, his old friend turned detective, but Buds can’t get into the mansion without a warrant, leaving Sam to confront the danger on his own terms. Daley, furious at Sam’s defiance, orders Curtis (Trife’s uncle) to strike at Sam’s family, while Curtis and Hugs plot to take control themselves. The violence spills into every corner of Sam’s life as Janette is dragged into danger, Mrs. Peel—Royston’s mother—finds herself embroiled in the maelstrom, and tragedy strikes at her door when Curtis and Hugs murder her.
