A new wing at Britannia Hospital is about to be unveiled as the Queen is scheduled to arrive, casting a spotlight on the institution’s inner workings. The hospital administrator, Potter, Leonard Rossiter, finds himself juggling a chaotic mix of demonstrators protesting an African dictator who is a VIP patient, protesting ancillary staff pushing back against the private patients’ elite demands, and a difficult head of the new wing, Professor Millar, Graham Crowden. Rather than cancel the royal visit, Potter ventures out to reason with the protesters and strikes a controversial compromise: the private patients will be ejected and, in return, the demonstrators will allow ambulances to flow into the hospital. Unknown to the crowd, these ambulances carry the Queen Mother and her entourage, turning a political truce into a chaotic moral test for everyone inside Britannia Hospital.
Mick Travis, Malcolm McDowell, is a reporter filming a clandestine documentary about the hospital and its dubious practices. He slips inside with the aid of a sympathetic nurse, Marsha Hunt, and begins to uncover Millar’s sinister scientific experiments, including the murder of a patient named Macready, Alan Bates. As mayhem escalates outside, Travis is killed and his head is used in a grisly Frankenstein-like experiment that spirals out of control, turning science into nightmare.
The protestors eventually breach the hospital’s defenses and interrupt Millar’s presentation of his so-called Genesis Project, a claim of perfected humanity. In front of an audience of royalty and commoners, Genesis is revealed to be a brain wired to machinery. The moment unfolds with a chilling, robotic speech that references a famous line from Hamlet:
