Millionaire Charles Deaver experiences a horrific car accident that grotesquely alters his appearance. Confronted with the specter of death, he finds himself desperate to escape his fate.

Deaver invests heavily in the research led by Dr. Nathaniel Leech, who is on a race against time to help his terminally ill wife, Miranda. Dr. Leech introduces Deaver to a groundbreaking serum aimed at regenerating dead cells but warns of its dire side effects.

Deaver's enforcers, including his right-hand woman Krendal, ambush Dr. Leech. In a brutal attack, she shoots him, injects him with the serum, and then locks him in a deserted tunnel system to monitor the effects.

Without the critical protein supplement, Dr. Leech's hunger spirals out of control, leading him to consume a rat and then a dog. This insatiable craving triggers his transformation into a monstrous hybrid creature.

A group of salvage workers, led by Vince Stoker, infiltrates a dilapidated hospital to scavenge for equipment. Among them is Stoker's niece, Sam, who unknowingly ventures into the same tunnel system that houses Dr. Leech.

As the salvage group explores, they inadvertently trigger Leech's monstrous evolution. His rampage leads to the gruesome death of Callum, with Sam desperately trying to warn Vince and Otis of the danger.

Deaver sends Krendal and Wilkes to retrieve Leech, unaware they've fallen into a trap. The duplicitous duo seals off all exits from the tunnel, trapping Stoker's crew inside.

The paths of the trapped salvage crew and Leech collide, leading to a horrifying encounter. Wilkes swiftly meets his demise at the hands of the beast, elevating the stakes for the remaining group.

Initially at odds, Krendal and Stoker's group unite against Leech. She reveals her true intentions as a double agent for the U.S. government, aiming to secure Leech's serum for supersoldier development.

Leech seeks revenge on Deaver, resulting in a shocking decapitation within a limousine. This ends the billionaire's grip on power and marks a pivotal point in the chaos that ensues.

In a desperate attempt to neutralize Leech, Vince and Krendal orchestrate a high-risk heist to lure the creature into the labyrinth of tunnels. Their plan is fraught with danger as they navigate the treacherous environment.

While executing their plan, tragedy strikes as Krendal suffers a gruesome injury while setting explosives. She sacrifices her safety to enable Vince to proceed with their perilous strategy.

Despite dismembering Leech with power tools, their triumph is short-lived as the detonation claims Otis's life. Only Vince and Sam manage to escape the ensuing chaos, leaving behind the horror.

In the aftermath, Vince awakens in a military hospital, haunted by the traumatic events. As a week passes, the grim reality sets in that he and the others have been infected by Leech's toxic serum.

As Vince lies tethered to an IV drip of the protein formula, he convulses in agony. The serum’s sinister influence awakens an unquenchable hunger within him, threatening to consume him entirely.
