Adjie Ario Bayu and Astrid Sigi Wimala, a married couple expecting a child, are gearing up for a move to Sydney for a fresh start. They are joined by their friends Alam Mike Lucock, Eko Dendy Subangil, and Jimi Daniel Mananta, who all head to the airport together. On the way, they pick up Adjie’s sister Ladya Julie Estelle, who initially refuses to come but is swayed after some persuasion.
Along the route, they encounter Maya Imelda Therinne, a pretty and distressed woman who says she has been robbed and cannot reach home. Moved by her plight, the group offers her a ride to a remote part of town where Maya’s family lives. At Maya’s house they meet Dara Shareefa Daanish, a mother whose youthful appearance hides a tense and unsettling demeanor, and her sons Adam Arifin Putra and Arman Ruly Lubis. Adam comes across as polite, while Arman remains withdrawn and stern. Dara subtly drugs Astrid’s drink, and the dinner that follows becomes a trap: the entire group is drugged, and Arman leads the others to a cellar beneath the house. Alam is subtly entangled in the tension—Maya initially tries to seduce him but he rejects her, prompting a vicious reaction that leaves Alam injured and Adam with a broken arm.
As Adjie and Astrid retreat upstairs to rest, the others are left to the house’s dark plan. Astrid, who is pregnant, soon realizes something is terribly wrong. Dara explains that the drug will push Astrid into labor prematurely, while a car outside signals help may be near. Astrid’s water breaks, and she gives birth to a boy in the chaos, clinging to her husband as danger closes in.
Meanwhile, in the cellar, Arman brutally slaughters Alam with a chainsaw. Ladya, driven by fear and resolve, disables Arman, frees Eko Dendy Subangil and Jimi, and tries to orchestrate an escape. Maya, armed with a crossbow, wounds Eko, and the group plunges into the forest as Adam pursues them. Ladya manages to slip away again, while Jimi falls to Adam’s cruel violence.
Back inside, Adjie ventures into a room filled with preserved baby cadavers and discovers Astrid dead, her neck pierced by a hairpin. Dara slides into the room and kills Adjie with a calculated strike before tightening the noose on his remaining resolve. Arman grabs Ladya, intending to force himself on her, escalating the household horrors to a deeper level of peril.
Police arrive suddenly in a patrol car, triggered by the earlier commotion. A briefing among four officers reveals a chilling find: a video clip from the film’s opening and photographs dated far in the past, including images of Adam, Arman, Maya, and a Dara whose date stamps stretch back to 1889. In the ensuing chaos, Arman is speared through the eye as Ladya fights to break free. A blackout plunges the house into darkness as the cannibal family stalks the lawmen.
The attackers claim the upper floors, and Maya is fatally shot by Dara at one point, while Dara herself continues to fight on for control of the weapon. The confrontation leaves many dead, including the officers and several members of the cannibal clan. Dara ultimately corners the surviving leader and kills him, then returns to the dying Maya to end her suffering with a swift death.
Arman’s fate comes quickly—he slashes a pursuing officer, and the battle spills into the corridors where Eko and Ladya add their own strikes. Adjie, now severely burned, joins Ladya in a final push. With Ladya’s help, Eko drives a stake in the fight and the two turn their attention to Adam, who is incapacitated in a fiery blaze. A brutal encounter ends with Adam’s head severed from his body.
Seeking safety for the newborn, Ladya and Adjie move the baby to a safer part of the house. Dara and Ladya’s confrontation continues, with Dara injuring Adjie’s shoulder in the struggle. Ladya seizes a pistol and turns it on Dara, shooting her, then finishes Dara off by strangling her with a necklace. The pair share a moment of fragile relief as Ladya promises to care for her nephew, even as Adjie, in his final moments, urges her to safeguard the child.
Morning light reveals Ladya driving away from the house, the infant safe in her arms. But a shocking twist remains: Dara crawls back into the scene, alive and violent, attacking Ladya once more. A tense chase ends with Ladya ramming Dara’s body against a tree and driving off, leaving Dara’s hand twitching in the dawn light as a grim reminder of what happened inside.
