Jeff Douglas and fellow agents Jimmy and Gene are ambushed while transporting mafia boss Raymond Manta out of a Mexican jail. The assault leaves them reeling, but Jeff survives the attack. This incident marks the White Hand's reach and pulls Jeff into their scheme.

The White Hand lies to Jeff, claiming his family is hostage and he must help smuggle a massive drug shipment to ensure their safety. Jeff is forced to become a one-time courier for the cartel, unaware of the deadly trap awaiting him. The cartel's leaders include Manta and four others.

The cartel's true intent becomes brutal: they have already murdered Jeff's wife and two young children and plan to kill him after the drug run. Jeff learns the personal cost of his involvement and his resolve hardens into revenge. This knowledge hardens his stance against the White Hand.

The drugs are delivered and the White Hand plans to kill Jeff with a car bombing in Las Vegas. Jeff's instincts kick in, allowing him to survive with only minor injuries, while the cartel believes him dead. He then learns that his family has been killed, which drives his desire for vengeance.

Officially censured but unofficially aided by agent Megan, the FBI's interest in Jeff resurges as he begins plotting to wipe out the White Hand cartel. He moves from a lackey in a dangerous system to a vengeful operative intent on dismantling the cartel. The pursuit marks a shift from survival to targeted retaliation.

Agent Megan secretly aids Jeff, driven by her own past. Off-the-books support helps Jeff obtain crucial FBI files that map out the cartel's operations. With this leverage, he starts moving toward decisive actions against the White Hand.

Using the stolen files, Jeff targets Helmut Vitch and kills him in his Las Vegas casino. The public nature of the hit alarms the cartel and exposes the White Hand's vulnerabilities. The strike marks Jeff's first major victory against the cartel.

In New Orleans, Jeff sabotages LaFleur's liquid-heroin deal by triggering a factory fire, crippling the cartel's distribution network. The move signals to Kowalski and Manta that Jeff is a serious threat and they must respond. Tensions within the White Hand rise as operations falter.

Later in New Orleans, Jeff finishes LaFleur at LaFleur's nightclub in full view of a satellite feed monitored by Manta and Kowalski. The killing destabilizes the cartel's leadership and heightens the pursuit of Jeff. The cartel's internal dynamics grow more volatile as a result.

Manta shoots and kills Kowalski at Kowalski's complex, angered by Kowalski's criticisms. Jeff retaliates with a savage beating and continues his mission to rescue Megan. The cartel fractures under the mounting pressure of Jeff's assault.

Jeff travels to Seattle and kills Hansel Lee in his heavily guarded mansion. The hit further destabilizes the cartel's leadership and triggers a chain of retaliatory moves. Megan arrives to speed up Jeff's getaway, tightening the clock on the cartel.

After Megan's involvement accelerates Jeff's escape, the couple are briefly captured by local cops and then ambushed by Manta's men. The pursuit becomes deadly as the cartel closes in on them. The tension peaks as they fight to stay alive.

At the FBI headquarters, Manta grabs a gun in a last-ditch escape attempt. Jeff drives him into a confrontation and plows him through a window, sending Manta to a fatal fall. The fall marks the collapse of the White Hand cartel and the completion of Jeff's vengeance.
