Sawyer’s harrowing journey reaches its climax after she finally escapes from the psychiatric hospital, only to discover that her nightmare isn’t over. Throughout the film, it’s revealed that the hospital was not just a mental health facility but a front for a shady operation that profited from involuntarily committing patients and draining their insurance money. Once Sawyer manages to kill David, her stalker who had murdered others and tried to control her, she believes she has finally found some measure of safety. She kills David with a blade she took from Violet, one of the other patients, during a tense confrontation in her cell, and after a brutal chase through the woods, she manages to escape the hospital.
However, her fight isn’t quite finished. She is then abducted by David, who knocks her unconscious and drags her into the woods. When Sawyer wakes up in his car trunk, she’s beside her mother’s decomposing body, a chilling reminder of her trauma. Despite the terror, she manages to open the trunk and run, injuring herself in the process. David catches up and severely injures her, smashing her ankle with a hammer and dragging her to a quiet spot where he expresses his twisted desire for them to be a family. Yet, Sawyer, still aware and resourceful, stabs him in the eye and slits his throat, finally ending his reign of terror.
The story then reveals that her ordeal was driven by more than just stalking; it was fueled by a corrupt hospital exploiting vulnerable patients for profit. With the help of Nate, a fellow patient-turned-informant who secretly recorded evidence of the hospital’s corruption, Sawyer’s story comes to light. Nate’s death at the hospital’s hands triggers a police investigation that uncovers the hospital’s sinister operation, leading to the arrest of the corrupt administrator. The truth about the insurance scam and illegal detentions is finally exposed, bringing justice.
In the final scene, set six months later, Sawyer appears to have moved forward—she’s been promoted and is enjoying lunch with a colleague. But her peace is short-lived. When she spots a man who looks like David across the restaurant, her mind spirals into panic. She prepares to attack him with a knife, but as he turns around, she realizes he’s just a stranger. Shocked and scared, she drops the knife and flees. The ending leaves us with a haunting image of her looking over her shoulder, suggesting that although she defeated her stalker, the trauma and fear still linger within her, keeping her forever haunted by the specter of her nightmare.
