Dr. Will Jennings, a research physician, has landed his major career break by patenting a new anesthetic drug. After presenting at a conference in a Seattle hotel, he is approached by a strange woman, Cheryl, who claims to be an admirer of his work. Cheryl tries to seduce him in the hallway, then pulls a gun and forces him into his hotel room, demanding a ransom for his young daughter, Abby. Meanwhile, Cheryl’s husband, Joe, has broken into Will’s luxurious home in Portland, Oregon, and confronts Will’s wife, Karen, who has just returned after picking Abby up from school. Joe intimidates Karen and takes Abby away, sending her to safety with his cousin, Marvin, while Karen fights back but is ultimately overpowered.
Marvin drives Abby to a remote cabin in the woods, where Abby’s asthma becomes a central fear—she relies on an inhaler to breathe. The illness triggers panic for Marvin, who calls Joe for help. Joe agrees to drive Karen to the cabin to administer the inhaler, blindfolding her for the trip. Karen manages to calm Abby and stop the attack, but Joe soon forces her back into the car. Back home, Karen confronts the danger but is held hostage as Joe asserts control. When their neighbor Joan stops by unannounced, Karen pretends to be having an affair with Joe to explain her absence. After Joan leaves, Karen secretly slashes Joe’s wound with a concealed scalpel and hides in the bathroom, where Abby secretly calls on Marvin’s cell phone. The call is cut short when Marvin recaptures Abby, and Joe re-enters the bathroom to keep Karen subdued.
At the hotel, Will manages to inject Cheryl with a paralyzing agent and then administers an antidote, bringing her back from the edge of death. Fearing for her life, Cheryl reveals the truth: she and Joe cooked this plan as revenge for their daughter Katie, who died during a brain operation that Will had helped with. Will explains that he was wrongly implicated by the head surgeon in Katie’s death, and Cheryl begins to sympathize with him, agreeing to botch the scheme.
The next morning, Cheryl accompanies Will to a bank to withdraw the ransom money while they remain under police scrutiny. Joe and Cheryl quarrel on the phone, revealing Joe’s plan to keep Abby and raise her as his own daughter. Will and Cheryl board a seaplane bound for Portland, with Joe and Karen arranging to exchange the money on a freeway below. Behind them, Marvin and Abby ride in a separate SUV. Karen makes a desperate move on Joe in the car, forcing a crash that interrupts the exchange. As the seaplane flies low over the highway, Will and Cheryl steer toward the scene and land the plane on the road, triggering a multi-car pileup. Marvin loses control of the SUV in the chaos, and Abby is briefly separated from her captors.
The confrontation between Will, Joe, and Cheryl escalates as Abby, still in danger, tries to find her parents. Joe presses Karen, while Marvin, Abby, and the rest of the group navigate the chaotic pursuit. In a tense turn, Karen shoots Joe to death during the standoff, ending his plan. Police and FBI agents arrive as Abby suffers another asthma attack, but Karen is able to treat her with the inhaler. Cheryl is captured by authorities, and the scene closes with Karen, Will, and Abby embracing each other, a relief after a harrowing ordeal.
