As the year 1899 draws to a close at Oxford University, the enigmatic Professor Edward Newgate presents a captivating case study on Eliza Graves, a patient grappling with the confines of female hysteria. Despite her fervent claims of sanity, the astute professor observes that most mental patients assert they are in control of their own rationality. This realization serves as the catalyst for Newgate’s eventual journey to Stonehearst Asylum, where he aims to establish his identity as Dr. Edward Newgate from Oxford.

Upon his entrance into the asylum, Newgate is immediately taken aback by the unconventional practices orchestrated by the charismatic superintendent, Dr. Silas Lamb. Rather than adhering to the standard methods of medication and confinement, Lamb encourages the exploration of patients’ delusions as potential sources of happiness. It is within this peculiar and liberating environment that Newgate finds himself increasingly attracted to Eliza, who becomes a significant figure in his life at Stonehearst.

As Christmas Eve nears, underlying tensions begin to escalate. A confrontation between Newgate and Mickey Finn, the leader of an armed group, ends in a precarious ceasefire. Just as Newgate prepares to partake in a festive toast proposed by Finn, Eliza intervenes. She implores him to escape the asylum, but Newgate is resolute, unwilling to leave without her.

As the plot thickens, Newgate soon uncovers the dark reality behind Lamb’s unorthodox methods. The true staff of the asylum, hidden away in the boiler room, unveil a chilling truth: Lamb and Finn have been drugging patients and inciting a revolt. Dr. Salt and Mrs. Pike warn Newgate about Lamb’s murky past as a surgeon, burdened by his wartime offenses, while Finn carries the horrific legacy of murdering his own family.

Entrusted with the care of Arthur Timbs, a volatile patient whose previous life in a traveling sideshow has left him broken, Newgate uses his observational prowess instead of sedatives to ease Timbs’ distress, referring to the scribbled names on the cell walls. However, Eliza remains hesitant to join forces with Newgate, sharing unsettling stories of Salt’s brutality.

In a desperate bid for freedom, staff members Paxton and Swanwick attempt to escape but fall victim to Finn’s gang. While sneaking into Lamb’s office to retrieve Salt’s notes, Newgate overhears Lamb and Finn’s nefarious scheming. Forced into administering shocking treatments on Salt, Newgate finds himself entangled in a web of lies and madness as Salt is declared cured, now under Lamb’s deceptive influence.

As the clock ticks toward midnight on New Year’s Eve, a horrific turn of events unfolds when Finn ruthlessly murders a female patient, her body unceremoniously removed by Timbs. Drawing on his instincts, Newgate tries to lace the champagne with chloral hydrate as a means to combat the chaos, but his plot is thwarted, and he finds himself cornered. Just when it seems his fate is sealed, Lamb is confronted with Newgate’s true intentions: a passionate desire to save Eliza from her chains. This revelation shocks Lamb to his core, leading him to reveal a heartfelt photograph of Eliza, only for Newgate to discover it bears the likeness of one of Lamb’s regrettable victims. The confrontation sends Lamb reeling, and chaos erupts as Finn’s cruelty incites the patients to rise against him.

The climax sees Finn meet his end at the hands of Arthur; his electrocution ignites an inferno that engulfs the asylum. Amidst the chaos, Eliza takes charge, leading fellow patients toward safety as Newgate seeks out Lamb, now a victim of his own remorse. Flashbacks unearth Lamb’s dark wartime past, revealing a layered character tormented by decisions borne from a misguided sense of mercy.

When silence finally returns, Newgate approaches Eliza, urging her to escape with him. However, she resists, pointing out their stark differences—he is deemed “normal,” while she feels forever trapped within the asylum’s confines. Unfazed, Newgate professes his love, disclosing a staggering secret that could alter their futures.

Months later, as Timbs stands guard at the institution’s gates, he prepares to welcome Graves’ husband and the seemingly altered Professor Newgate. Yet, it becomes chillingly clear that this man is a fraud, a doppelgänger with a penchant for deception. Meanwhile, Lamb, engaged in a game of chess with Salt, masks his amusement as Eliza and her impostor partner—posing as the Lambs—are shown enjoying a deceptively peaceful existence in beautiful Tuscany, Italy.
