Frank Castle [Dolph Lundgren] is a former undercover police detective whose wife Julie and two daughters were killed five years earlier in a Mafia car bomb meant for him, leaving him to disappear from public life. Castle has since become the city’s infamous vigilante—known only as The Punisher—and now makes his home in the city’s labyrinthine sewer system, where he has taken out roughly 125 mobsters (not counting henchmen) over the past five years. His work is marked by his skull-engraved throwing knives, a grim symbol of his single‑minded war on organized crime, and his one constant ally is Shake [Barry Otto], a stage performer turned derelict who speaks in rhyme and offers a strange, gleaming rationality to Castle’s relentless crusade.

The underworld has grown too weakened by the Punisher’s guerrilla tactics for its own good, and kingpin Gianni Franco [Jeroen Krabbé] is dragged out of retirement as he seeks to reassemble the fractured mob. It’s revealed that Franco personally ordered the car bombing that killed Castle’s family, a revelation that sets him on a collision course with a power they neither anticipated. Franco’s plan is to unify the scattered families under a single, ruthless control, but his ambitions draw the attention of Asia’s most formidable crime syndicate, the Yakuza, led by Lady Tanaka [Kim Miyori]. To bend the Mafia to their will, the Yakuza kidnap the children and hold them for ransom, triggering a race against time that pushes the Punisher to the edge of his own ethics and violence.

Shake pleads with the Punisher to intervene, arguing that the children face a future of exploitation if the mob refuses to concede. The Punisher escalates his assault on Yakuza interests, striking fear into the criminals and warning that each day the children remain missing costs them money. The Yakuza respond by capturing the Punisher and Shake and attempting to torture them into submission, but Castle breaks free and resolves to mount a direct rescue. He frees the children he can reach and commandeers a bus to shepherd them toward safety, though Tommy Franco, Gianni Franco’s son, has already been taken to a Yakuza headquarters.

The rescue attempt leads to a police interception of the bus, and Frank is arrested after he completes his mission to save the children. While in custody, Castle reunites with one of his old partners, who warns that his history of killing will almost certainly bring him to the edge of execution. Frank remains philosophically detached, insisting that he is already dead in a sense—no longer a man, but a symbol of vengeance. Franco breaks him out to help rescue his own son and to crush Lady Tanaka’s bid for power; he even expresses regret that the bomb ended Castle’s family, while Frank makes it clear that Franco will pay with his life once the Yakuza’s threat is eliminated.
