In the night of 1966, a young patient named Tammy is brutally attacked and killed by an unseen force at North Bend Psychiatric Hospital. Her mysterious death sets the stage for the supernatural events that follow in the hospital.

The next morning, Kristen, a troubled young woman, is discovered setting fire to an abandoned farmhouse and is subsequently taken to the psychiatric hospital. She arrives bruised and cut, unaware of the terrifying events that await her within.

Kristen is assigned to Tammy's old room and meets other patients including Iris, Sarah, Emily, and Zoey. She struggles to adapt to the hospital environment and is given medication to help her sleep.

While trying to retrieve her blanket during the night, Kristen finds a broken charm bracelet on the floor. This moment marks the beginning of the eerie supernatural events that will haunt her.

Kristen meets with Dr. Stringer, who questions her about the fire and the notes on her hand. Despite his probing inquiries, Kristen struggles to recall her memories and insists she is not mad.

That night, Kristen tries to escape the ward but is caught by an orderly and returned to her cell. Her fear escalates as she starts to witness horrifying visions, including a disfigured figure looking at her through her door.

During a shower, Kristen is suddenly attacked by the same disfigured figure she had seen earlier. When she reports this to the nurse, she is dismissed and subjected to intense electroshock therapy, worsening her plight.

After a hypnotherapy session, Iris mysteriously disappears, only to later be found dead, murdered by the ghostly figure. This alarming event intensifies Kristen's determination to uncover the truth behind the terror in the hospital.

Kristen discovers Iris's sketchbook and finds drawings connected to a girl named Alice, alongside the ghostly figure. The name ignites Kristen's quest for the truth, revealing that Alice Hudson was once a patient at the hospital.

Kristen confronts Dr. Stringer, passionately arguing that the spirit of Alice Hudson is haunting the facility. However, Dr. Stringer remains skeptical and advises Kristen to search for her own answers.

Desperate to escape, Kristen enlists Emily to help her. Their attempt is thwarted, and while Kristen makes a desperate run to the entrance, Alice's ghost appears, causing her to pass out and wake up back in her room.

On her way to meet Dr. Stringer, Sarah is brutally attacked and killed by Alice's ghost. This tragedy causes panic among the remaining girls, who must confront the ghost's vengeful nature.

Overwhelmed by fear and despair, Emily, aided by Alice’s ghost, takes her own life. This grim act underscores the psychological torment the girls endure and raises the stakes for Kristen's survival.

Kristen attempts a final escape by holding Zoey hostage, only to be thwarted again. As she breaks free and confronts Alice's ghost, a climactic battle ensues, revealing shocking truths about her identity.

In a shocking revelation, Dr. Stringer uncovers Kristen's true identity as Alice Hudson, revealing the traumatic past that led to her multiple personality disorder. The revelation culminates in Alice’s ghost throwing herself out of the window, leading to Alice's awakening.
